Erişkin İdiopatik Skolyozon Cerrahi Tedavisinde Cotrel –Dubousset İnstrümentasyonu Uygulaması Sonuçları

Abstract

The results of cotrel-dubodusset instrumentation in the treatment of adult idiopathic scoliosis. ✓ Treatment of idiopathic scoliosis in adults are more difficult with regard to adolescent patient. Generally curve is rigid and can not be easily corrected as adolescent scliosis. Long and short term complication rates are high. In this study, the results of 11 adult patients whom had pain and deformity complaints and treated with Cotrel-Dubousset Instrumentation are examined. Average follow-up period was 26 (14-50) months. All of the patients were male. Mean age was 27.6 (22-36) years. 5 of the patients had King type II, 5 had King type III and 1 had King type IV curves. When all the patients were induced preoperative Cobb angle which was averagely 56.6° ± 10,7° was corrected to 27.6° + 10,5°. The best results were obtained in King type II curves and satisfactory correction in lumbar curves weren't obtained in king type II curves. Pain disappered in 8 of the patients. Neurologic defficiency wasn't seen in any of the patients. In view of these data it is suggested that Cotrel-Dubousset Instrumentation can be an alternative method in the treatment of adult scoliosis. Erişkin hastalarda idiopatik skolyozun cerrahi tedavisi, adölesan hastalara nazaran oldukça güçtür. Erişkinde eğrilik genellikle daha rijittir ve adölesandaki gibi kolayca düzeltilemez. Kısa ve uzun dönem komplikasyon oranı yüksektir. Bu çalışmada idiopatik skolyozu olan, ağrı ve deformite yakınması ile gelen ve cerrahi korreksiyon İçin Cotrel-Dubousset İntrümentasyonu kullanılan 11 erişkin hastanın cerrahi sonuçları incelendi. Ortalama takip süresi 26( 14-50 ay) aydı. Hastaların tamamı erkekti. Yaş ortalaması 27,6 (22-36 yaş) idi. Hastalardan 5'i King Tip II, 5'i King Tip III ve Fi King Tip İV eğriliğe sahipti. Tüm hastalar dahil edildiğinde preoperatif Cobb açısı 56,6° + 10.7° iken,'ortalama 27,6° ± 10,5° düzeltme temin edildi. En iyi sonuçlar King Tip III eğriliklerde elde edilirken, King Tip II eğriliklerde lomber bölgede tatmin edici düzeltme sağlanamadığı saptandı. Hastaların 8'inde ağrı tamamen ortadan kalktı. Nörolojik defisite hiç bir hastada rastlanmadı. Bu verilerin ışığı altında Cotrel-Dubousset lnstrümentasyonunun erişkin skolyozu olan hastaların cerrahi tedavisinde alternatif tedavilerden biri olduğu görüşü elde edildi.
