Abstract

Hypertensive Crises and Treatment
t/ Hypertensive crises occur less than 1 percent of patients with hypertension and dias-tolic blood pressure is generally above 120 to 130 mmHg. If hypertensive crises are not treated immediately, they lead norologic, cardiac, renal or retinal end-organ dama¬ge. Hypertensive crises are classified as hypertensive emergencies in the presence of acute endorgan damage end-organ damage. Hypertensive emergencies must be treated in the intensive care unit. Hypertensive urgencies are treated with oral antihypertensi¬ve drugs. Hypertensive encephalopathy is the most important situation in hypertensive emergencies. Sodium nitroprusside, labetolol, diazoxide, nitrogliserin, nifedipine and captopril are used frequently in the treatment of hypertensive crises.


Hipertansif acil durumlar; hipertansiyonlu hastaların %1'inden daha azında görülen, diyastolik kan basıncının sıklıkla 120-130 mmHg üzerinde olduğu ve hemen tedavi edilmemeleri halinde nörolojik, kardiyak, renal veya retinal organ hasarına yol açabilen durumlardır. Hipertansif acil durumlar; akut organ hasarı varlığı ile birlikte olan hipertansif gerçek acil durumlar ve organ hasarı bulunmayan hipertansif ivedi durumlar olarak sın ulandırılırlar. Hipertansif gerçek acil durumlar hastanede yoğun bakını ünitesinde parenteral antihipertansif ilaçlarla tedavi edilirken hipertansif ivedi durumlar ise oral antihipertansiflerle tedavi edilir. Hipertansif ensefalopati en önemli hipertansif gerçek acil durumdur. Tedavide en sık kullanılan ilaçlar sodyum nitrop-russid, labetolol, diazoksid, nitrogliserin, nifedipin ve kaptoprildir.
