Miksiyon Sonrası Görüntülerin Eklenmesi ile Elde Edilen Yeni Renogram Eğri Paterninin Obstrüktif Böbrek Patolojisi Tanısına Katkısı‡

Abstract

A New Renogram Obtained by Adding Post-Micturation Images on the Diagnosis of Obstructive Uropathy The aim of this study was to evaluate the diagnostic input of the renal washout percentage and the extended renogram curve which was obtained by quantifying the data of post voiding dynamic images. Sixtysix kidneys of 54 patients (11 female and 43 male; age 1 month- 72 years ) with Tmax1/2 values >10 minutes (min) in the diuretric renography (group 1: Tmax1/2 > 20 min and group 2: Tmax1/2 between 10-20 min) were enrolled for the study. The kidneys in group 1 were also divided into 2 groups according to differential renal functions (group 1A >40%; group 1B <40%). Just after the first dynamic imaging, two additional post-voiding dynamic images using the same acqusition parametres (15 sec/image; 128*128), as the first imaging, were obtained for all patients. The original (R30) and extended renogram curves (Rek) were generated in the excell programme by using the background and decay corrected total counts of each kidney obtained from the data of dynamic acquisitions. The renal washout percentages at the first 30 minutes (min) acquisition (BY30), the first (BY^) and the second (BYek) post-voiding imaging were also calculated. The washout percentage of <50% was accepted as obstruction. In group 1 (45 kidneys), the evaluation of wash-out percentages in the first 30 minutes dynamic images, the first and the second post-voiding images revealed obstruction in 43, 34 and 22 kidneys respectively. While the the original renogram curve (R30) showed obstruction in 43 kidneys, the extended renogram curves (Rek) showed obstruction in 31 kidneys. The difference between BY30, BY^ve BYek2 values in group 1 were found statistically significant (p<0,001). In group 2 (21 kidneys), the evaluation of wash-out percentages in the first 30 minutes dynamic images, the first and the second post-voiding images revealed obstruction in 3, 1 and 1 kidneys respectively. Thirteen kidneys of R30 and 10 kidneys of Rek showed obstructive renogram patern. The difference between BY30 and BYek1 (p<0,01), BY30 and BYek2 (p<0,001), BYek1 and BYek2 (p<0,001) values in group 2 were found statistically significant. Eleven kidneys in group 1A and 10 kidneys in group 1B revealed obstruction in the evaluation of BYek values (p<0,01 ve p<0,05). During the study period, 11 kidneys in group 1 and 1 kidney in group 2 were operated in which 8 kidneys revealed UP narrowing, 1 kidney revealed compression of aberant fibrotic band, 1 kidney revealed aberant renal artery and 2 kidneys revealed ureterocel. The scintigraphic analysis of only 8 kidneys supported obstruction. Seven kidneys came for control scintigraphy and non of them demonstrated >10% difference at split renal function during follow up. Obtaining the post-voiding images in a dynamic format allowed us to perform extended renogram and to calculate the renal washout percentages. Although the number of surgically diagnosed patient is not sufficient to get a definitive conclusion, the renal washout percentages and the extended renograms will contribute additional information in the diagnosis of obstructive uropathy. Çalışmadaki amacımız, miksiyon sonrası alınan dinamik görüntülerden elde edilen sayısal verilerle, uzatılmış bir renogram eğrisi elde etmek ve hesaplanan böbrek boşaltım yüzdesi ile birlikte sintigrafik tanıya katkısını değerlendirmektir. Tmaks1/2 değeri >10 dakika (dak.) olan (grup 1: Tmaks1/2 >20 dak ve grup 2: Tmaks1/2 10-20 dak) 54 hastaya (66 böbrek) 30 dakikalık dinamik çekim sonrası, aynı çekim parametrelerinde 2 dakikalık iki ek dinamik çekim alındı. İlk dinamik çekim (R30), miksiyon sonrası alınan birinci (Rek1) ve ikinci (Rek2) dinamik görüntülerden, zemin aktivitesi düzeltilmiş ve radyoaktif yarılanma formülü uygulanmış son böbrek sayımları elde edildi. Bu veriler kullanılarak bilgisayarda excell programında orijinal (R30) ve uzatılmış (Rek) renogram eğrileri ile böbrek boşaltım yüzdeleri (maksimum-minimum/maksimum aktivite) sırasıyla BY30, BYek1 ve BYek2 hesaplandı. Obstrüksiyon tanısında görsel değerlendirme bulguları ile böbreklerin boşaltım yüzde değerleri karşılaştırıldı. BY değeri <%50 obstrüksiyon kabul edildi. Grup 1 (N=45)'de; ilk dinamik çekim sonunda 43 böbreğin, birinci ve ikinci ek çekim sonunda sırasıyla 34 ve 22 böbreğin boşaltım yüzde değeri obstrüksiyonu desteklemekte iken, R30 eğri paterninde obstrüksiyonu düşündüren 43 böbrekten, 31 böbrekte uzatılmış eğride obstrüktif patern mevcuttu. Verilerin istatistiksel değerlendirmesi sonucu, grup 1'de BY30, BYek1 ve BYek2 değerleri arasındaki fark anlamlı idi (p<0,001). Grup 2 (N=21) de; ilk dinamik çekim sonunda 3 böbrekte, birinci ve ikinci ek çekim sonunda ise 1 böbrekte boşaltım yüzde değerleri obstrüksiyonu desteklemekte iken, R30 eğri paterninde obstrüksiyon düşündüren 13 böbrekten, 10 böbrekte uzatılmış eğride obstrüktif patern mevcuttu. İstatistiksel olarak grup 2'de BY30 ve BYek1 (p< 0,01), BY30 ve BYek2 (p< 0,001), BYek1 ve BYek2 (p<0,001) değerleri arasındaki fark anlamlı idi. Tmaks1/2 değeri >20 olan böbrekler, total fonksiyona katılım oranlarına göre gruplandırıldı-ğında, grup 1A (>%40)'da, 29 böbrekten 11'inde ve grup 1B (<%40)'de 14 böbrekten 10'un-da BY değerleri obstrüksiyonu desteklemekteydi (P<0,01 ve P<0,05). Çalışma süresince grup 1'de 11 böbrek ve grup 2'de 1 böbrek obstrüktif böbrek hastalığı ön tanısı ile operasyona alındı. Operasyon sonucu 8 böbrekte UP darlık, 1 böbrekte aberan bant, 1 böbrekte aberan damar ve 2 böbrekte ureterosel tespit edildi. Sintigrafik olarak yalnızca 8 böbrekte obstrük-siyon sonucuna varılmış idi. Operasyona alınmayan ve sintigrafik takibe gelen 7 böbrekten hiçbirisinde, total fonksiyona katılım oranlarında %10'dan fazla azalma tespit edilmedi. Rutinde statik olarak alınan ek görüntülerin bu çalışmada dinamik formatta kaydedilmesi, böbrek boşaltım yüzdesi hesaplanmasına ve uzatılmış renogram eğrisi elde edilmesine olanak sağlamıştır. Operasyona alınan ve obstrüksiyon tanısı kesinleşmiş hasta sayısı yetersiz olmasına rağmen, ek çekimlerden elde edilen böbrek boşaltım yüzdesi ile uzatılmış renogram eğri paterninin obstrüksiyon ayırıcı tanısında ek katkı sağlayacağı bu çalışmada gösterildi.
