Özofagus kanserinde transtorasik özofagus rezeksiyonu

Abstract

Özofagus kanserinin tedavisine yönelik yapılan, özofagus rezeksiyon ve rekonstrüksiyonunda değişik yöntemler kullanılabilir. Tümörün lokalizasyonu ve evresi, tümörlü segmentin uzunluğu, hastanın yaşı, eşlik eden hastalıklar ve daha önce geçirilmiş gastrointestinal ameliyatlar yöntem seçiminde etkilidir. Transtorasik yaklaşım olarak adlandırılan ve torakotominin gerekli olduğu yol en sık kullanılan girişim yoludur. Transtorasik girişim yolu laparotomi ve servikal kesi ile değişik şekillerde kombine edilerek özofagus alt, orta ve üst bölge tümörlerinin rezeksiyon ve rekonstrüksiyonu rahatlıkla yapılabilir. Bu yöntemin en önemli avantajı mediastinal lenf bezi diseksiyonunun yapılabilmesidir. Torasik inlet eksplorasyonu gerekli görülür ise veya trakea invazyonu şüphesi var ise transtorasik girişim gereklidir. J. Exp. Clin. Tumor location and stage, length of tumor segment, age of patient, comorbidities, and history of gastrointestinal surgery of the patient are effective for selection of method. Transthoracic approach is the most commonly way when thoracotomy is required. Transthoracic approach can be combined with laparotomy and servical incision in different ways; thus, resection of upper, middle and lower esophagus and reconstruction can be performed easily. Mediastinal lymph node dissection is the most important advantage of its.
