Abstract
SUMMARY
A Case of Multiple Myeloma Associated With Hyperthyroidism
A 66 year old female patient who had been followed up for hypertension and coronary artery disease for 6 years, complained of back pain for 4 months which prevented her from walking. As CT was made to find out the origin of a possible metastatic cancer, disseminated small medullary lytic defects were found in skull, clavicle, scapula, the ribs and the spine. Bone narrow biopsy revealed plasma cell infiltration. No M - protein was found in protein electrophoresis. Serum end urine electrophoresis was normal and Bence - Jones proteinuria was negative. Clinically the patient was hyperthyroid. Laboratory revealed a highly T4 and low TSH level. A toxic nodule was found in the right upper pole in thyroid scanning. The patient was given 4 occasions of CMP cure and PTU and was accepted as being in remission regarding last haematologic and immunologic findings. The patient is presented as a case of non-secretory multiple myeloma asociated with hyperthyroidism.

Altı yıldır esansiyel hipertansiyon ve koroner arter hastalığı (angina pectoris] tanılarıyla izlenmekte olan 66 yaşında kadın hastada 4 aydır yürü¬me kısıtlılığına yol açan bel ağrısı nedeniyle kanser metastazı taraması amacıyla yapılan tüm vücut bilgisayarlı tomografisinde tüm kafa kemikle¬rinde, klavikula ve skapulada, tüm kosta yapılarında ve vertebral korpuslarda milimetrik boyutlu, düzgün kenarlı, yaygın, medüller, litik defektler izlendi. Kemik iliği biyopsisinde plazmositoın infiltrasyonu1 bulundu. Hasta¬da protein elektroforezinde M proteini mevcut değildi. Serum ve idrar immünelektroforezi normal bulundu. Bence-Jones proteini negatif idi. Ol¬guda klinik ve laboratuar olarak hipertiroidi bu tabloya eşlik etmekte idi. T4 yüksek, TSH düşük bulundu. Tiroid sintigrafisinde sağ lob apeksinde toksik nodüler guatr saptandı. Dört kez CMP kürü uygulanan ve PTU veri¬len hasta son yapılan hematolojik ve immünolojik tetkiklerinde remisyonda kabul edildi. Hasta, hipertiroidi ile assosiye non - sekretuar bir multipl myelom olgusu olarak takdim edilmektedir.
