İleostomili total kolonik aganglionozisli hastada beslenme yönetimi: vaka sunumu

Abstract


Uzun segment Hirschsprung hastalığı (USHH) veya total kolonik
aganglionozis (TKA) Hirschsprung hastalığının (HH) en nadir formlarındandır. Yakın
geçmişe kadar mortalitesi yüksek olan hastalıklar arasındaydı. Klinik
belirtilerin iyi anlaşılması, radyolojik görüntülemeler ile erken tanı konulabilmesi,
uygun cerrahi uygulama ve yeterli beslenme destek yöntemlerinin geliştirilmesi
ile mortalite ve morbidite zaman içinde azaltılabilmiştir.   
Yazımızda
batın distansiyonu ve safralı kusma nedeni ile değerlendirilen matür yenidoğan
erkek hasta sunulmaktadır. HH şüphesi ile rektal biyopsi yapılan ve ganglion
hücresi görülmeyen hastaya transanal endorektal pull through (TEPT) yapılması
planlandı. TEPT sırasında yapılan biyopsilerle TKA saptanan hastaya, ileal pullthrough
ve ileostomi yapıldı. Operasyon sonrası ileostomiden kayıp nedeniyle metabolik
bozukluklar gelişen ve kilo alımı olmayan hasta metabolizma bölümü ile birlikte
takip edildi. 
TKA nedeni ile definitif
ameliyatı yapılan ileostomisi olan hastalar sıvı elektrolit dengesizliği nedeni
ile yüksek morbiditeye sahiptir. Mortalite ve morbiditenin azaltılabilmesinde,
beslenme desteğinin ve multidisipliner yaklaşımın önemini belirten olgu sunumu
hazırlandı.

Nutritional management in a total colonic aganglionosis with ileostomy: case report

Abstract

Long segment Hirschsprung disease (LSHD) or total colonic
agangliosis (TCA) is one of the rarest forms of Hirschsprung disease (HD). It
was among high mortality diseases since near future. Better understanding of
clinical symptoms, early diagnosis with the help of radiologic imaging,
development of proper surgical techniques and methods for dietary
supplementation have decreased mortality and morbidity in time. Her we present
a mature newborn male patient suffering from abdominal distention and vomiting.
HH was suspected and agangliosis was determined in rectal biopsy specimens.
Transanal endorectal pull through (TEPT) was performed and TCA was diagnosed
from biopsy specimens taken during TEPT. Ileal pull through and ileostomy was
performed and metabolic problems occurring due to loss from ileostomy after
operation was managed with collaboration of department of metabolic diseases. Patients
who are operated for definitive operations for TCA and who have ileostomy are
prone to water-electrolyte imbalance and have high morbidity. In order to
decrease mortality and morbidity, enteral nutrition and multidisciplinary
approach are important.
