Elektif cerrahide rutin preoperatif koagülasyon testlerinin yeri

Abstract

Amaç: Ülkemizde hemen tüm merkezlerde preoperatif koagülasyon
testleri rutin olarak yapılmaktadır. Bu testler içerisinde, protrombin zamanı
(PTZ), parsiyel protrombin zamanı (aPTT), ve uluslararası normalize değer (INR)
yer almaktadır. Bu retrospektif çalışmada biz bu laboratuar testlerinin uzamış
değerlerinin intraoperatif ve postoperatif kanama ile korelasyonunu araştırdık.

Gereç ve Yöntem: Çalışma grubuna, 2015 ve 2017 yılları arasında, Ankara
Medical Park Hastanesi KBB Kliniğinde tonsillektomi, adenoidektomi ve
septoplasti operasyonları yapılmış olan 250 hasta dahil edildi. Operasyon
sırasında ve operasyonu takip eden 45 gün içerisinde kanaması olan hastalar
ayrıca değerlendirildi. Operasyon öncesi ilaç kullanımı ve kronik hastalığı
olan hastalar çalışmaya dahil edilmedi.

Bulgular: Çalışmaya alınan hastalardan preoperatif PTZ değeri
uzamış olan 32 hastadan (%12,8) sadece 2’ sinde (%6,2) hafif intraoperatif
kanama izlendi. aPTT değeri uzamış bulunan 15 hastadan (%6) yalnızca 1’inde (%6,6)
hafif intraoperatif kanama izlendi. Bu hastalardan hiçbirinde 45 gün içerisinde
postoperatif kanama olmadı. INR değeri yüksek olan 45 hastadan (%18), 5 hastada
(%11) intraoperatif hafif kanama izlenirken, 2 hastada (%4,4) postoperatif 45
gün içerisinde kanama izlendi.







Sonuç: Bu bulgulara dayanarak, preoperatif koagülasyon
testlerinin, kanama eğilimi olan hastaları tespitte düşük duyarlılıkta olduğu
ve şüphe uyandıran bir öykü yoksa yapılmasının gereksiz olduğu sonucuna
varılmıştır.

Evaluation of preoperative coagulation tests for elective surgeries

Abstract

Aim: Preoperative coagulation screening tests are routinely
performed in our country. Screening tests include PT, prothrombin Time; PTT,
partial thromboplastin time; and INR, international normalized ratio. We
investigated the predictive value of these tests for intraoperative and
postoperative bleeding.

Material and Method:
We reviewed the records of 250 elective tonsillectomy,
adenoidectomy and septoplasty operations performed at the Ankara Medical Park
Hospital between the years of 2015and 2017. We also reviewed the patients who
experienced intraoperative or/and postoperative bleeding. Those who have bleeding
disorders are excluded from the study.

Results: 32 (12.8%) patients had preoperative prolonged PT values
but only two (6.2%) of these patients had intraoperative bleeding. 45 (18%)
patients who had prolonged preoperative INR values, only five (11%) experienced
bleeding intraoperatively. Two (4.4%) patients with prolonged INR values
experienced light bleeding during the 45 days subsequent to surgery. 15 (6%)
patients had prolonged preoperative PTT values, only one (6.6%)bexperienced
light bleeding during surgery.







Conclusion: This
study shows that preoperative coagulation screening tests have low predictive
values for intraoperative and postoperative bleeding and should only be
performed if there is suspected medical history for coagulation disorders.
