Diagnostic value of haematological parameters in neonatal sepsis cases

Abstract

Aim: In this study, we aimed to
investigate whether haematological parameters could be used in the early
diagnosis of neonatal sepsis in neonates diagnosed with sepsis clinically
.

Material and Method: This study was performed retrospectively from the patient records of 138
newborn infants who were diagnosed with neonatal sepsis whom admitted to Konya
Beyhekim State Hospital Children's Health and Diseases Clinic, Neonatal
Intensive Care Unit, between January 2014 and January 2016. Gender, type of
delivery (normal vaginal delivery or cesarean section), birth weight, body
weight and age at admission, gestational age, length of stay in hospitals,
accompanying complaints/symptoms, risk factors in the noenate mother were
recorded from patient record files.

Results: It was determined that
138 (19.43%)
newborn
infants
of the 710
admitted to our unit were diagnosed with neonatal sepsis diagnosis. Of these
patients, 82 (59.4
%) were male. While 127 patients were diagnosed with
clinical sepsis, 11 (7.97
%) of them were haemoculture-proven. The mean birth
weight of the patients were 3082,14 ± 423,90 grams and 12 (8,69
%) patients
were below 2500 grams. The mean C-reactive protein (CRP) value of the patients
was 19.01±15.30 mg/dl and the mean procalcitonin value was 7.75±6.72 ng/ml. CRP
level was high in 81.88% of the patients. White blood cell (WBC), neutrophil
count, neutrophil/lymphocyte ratio (NLR), mean platelet volume (MPV) and
platelet distrubition width (PDW) were statistically higher in the patient
group than in the control group.

There was no statistically
significant difference between the patient group and the control group
according to platelet/lymphocyte ratio (PLR).

Conclusion: This study revealed that haematological parameters are
still very valuable in the early diagnosis of newborn infants diagnosed with
sepsis.

Yenidoğan sepsis olgularında hematolojik parametrelerin tanısal değeri

Abstract

Amaç: Bu çalışmada klinik olarak sepsis tanısı
konulan yenidoğanlarda, hematolojik parametrelerin yenidoğan sepsisinin erken
tanısında kullanılıp kullanılamayacağını araştırmayı amaçladık.

Gereç ve Yöntem: Bu
çalışma; Ocak 2014-Ocak 2016 tarihleri arasında Konya Beyhekim Devlet Hastanesi
Çocuk Sağlığı ve Hastalıkları Kliniği, Yenidoğan Yoğun Bakım Ünitesi’ne yenidoğan
sepsisi ön tanısı yatırılmış
olan 138 yenidoğan bebek üzerinde kayıtlarının geriye dönük olarak
incelenmesi sonucu elde edilen verilerle yapıldı.
Hasta
dosyasından; cinsiyet, doğum şekli (normal vajinal doğum, sezaryen), doğum
tartısı, hastaneye yatış tartısı, hastanın kaçıncı gün yatırıldığı,
gestasyon
yaşı,
yattığı gün sayısı, eşlik eden diğer yakınmalar, bebekte ve annede
risk etmenler kaydedildi.

Bulgular: Ünitemize yatırılan
710 yenidoğan bebekten 138’inin (%19,43) neonatal sepsis tanısı ile tedavi
aldığı saptandı. Bu hastaların 82’si (%59,4) erkek idi. Yüz yirmi yedi hastada
klinik sepsis tanısı konulurken hastaların 11’i (%7,97) hemokültür ile
kanıtlanmış sepsis idi. Hastaların ortalama doğum ağırlıkları 3082,14±423,90 gr
olup, 12 (%8,69) hasta 2500 gramın altında idi.
Hastaların ortalama C-reaktif  protein (CRP) değeri
19,01±15,30 mg/dl, ortalama prokalsitonin değeri 7,75±6,72 ng/ml saptandı. CRP
düzeyi hastaların %81,88’inde yüksek idi. Beyaz küre sayısı (WBC), nötrofil
sayısı (Neu), nötrofil/lenfosit oranı (NLO), ortalama
trombosit hacmi (MPV),  trombosit dağılım
genişliği (
PDW) açısından hasta grubunda kontrol grubuna göre
istatistiksel olarak yüksek tespit edildi. Platelet/lenfosit oranı (PLO) düzeyi
açısında hasta grubu ile kontrol grubu arasında istatistiksel olarak anlamlı
fark bulunmadı.







Sonuç: Bu
çalışma klinik olarak sepsis tanısı konulan yenidoğanlarda, hematolojik
parametrelerin yenidoğan sepsisinin erken tanısında
halen
çok kıymetli olduğunu bir kez daha ortaya koymuştur.
