A rare case of ovarian pathology: diffuse large B cell lymphoma of over

Abstract

Primary ovarian non-Hodgkin lymphoma is a challenging diagnosis due to its rarity, often leading to delayed treatment, wrong diagnosis, and inappropriate therapy. The patient in this case presentation presented with abdominal pain as the initial symptom. Subsequent abdominal computed tomography revealed a bilateral heterogeneous ovarian mass measuring approximately 6x10x10 cm on the left side and 14x10x7 cm on the right side. An intestinal mass lesion was observed in the ileal ans, causing diffuse wall thickening of ~2 cm at its widest point and focal lumen widening in an ~11 cm segment (lymphoma?). A transvaginal ultrasound was also performed, revealing two cystic masses, one measuring 10 cm and the other 12 cm, with dense and heterogeneous contents filling the uterine cavity and pelvis. Subsequently, a total abdominal hysterectomy, bilateral salpingo-oophorectomy, bilateral pelvic lymph node dissection, and small bowel resection and anastomosis were performed.

Nadir bir yumurtalık patolojisi vakası: overin yaygın büyük B hücreli lenfoması

Abstract

Primer over non-Hodgkin lenfoma, nadir görülmesi nedeniyle tanı koyması zor bir hastalıktır ve sıklıkla tedavinin gecikmesine, yanlış tanıya ve uygun olmayan tedaviye yol açar. Bu vaka sunumundaki hasta, ilk semptom olarak karın ağrısı ile başvurmuştur. Daha sonra yapılan karın bilgisayarlı tomografide, sol tarafta yaklaşık 6x10x10 cm ve sağ tarafta 14x10x7 cm boyutlarında bilateral heterojen over kitlesi saptanmıştır. Ileal ans'ta bağırsak kitlesi lezyonu gözlendi; bu lezyon, en geniş noktasında ~2 cm'lik yaygın duvar kalınlaşmasına ve ~11 cm'lik bir segmentte fokal lümen genişlemesine neden oluyordu (lenfoma?). Transvajinal ultrason da yapıldı ve uterus boşluğunu ve pelvisi dolduran yoğun ve heterojen içerikli, biri 10 cm, diğeri 12 cm boyutlarında iki kistik kitle saptandı. Ardından, total abdominal histerektomi, bilateral salpingo-ooforektomi, bilateral pelvik lenf nodu diseksiyonu ve ince bağırsak rezeksiyonu ve anastomoz yapıldı.
