Anesthesia management of a pregnant woman with a history of postdural puncture headache and spinal muscular atrophy carrier status

Abstract

Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disorder characterized by progressive proximal muscle weakness and atrophy. Anesthetic management in obstetric patients with neuromuscular conditions requires careful planning, particularly in the presence of previous postdural puncture headache (PDPH) and concurrent respiratory symptoms. A pregnant woman with SMA carrier status was scheduled for cesarean section due to repeat cesarean delivery, gestational hypertension, and premature rupture of membranes. Her obstetric history included a missed abortion at 12 weeks and PDPH following spinal anesthesia during a prior cesarean section. She also presented with a five-day history of cough and postnasal drip. Considering her refusal of spinal anesthesia and the potential risks associated with general anesthesia, epidural anesthesia was administered, avoiding neuromuscular blocking agents and tracheal intubation. Epidural anesthesia may represent a safe and effective alternative for cesarean delivery in selected obstetric patients with SMA carrier status and prior PDPH history.

Postdural ponksiyon baş ağrısı öyküsü ve spinal musküler atrofi taşıyıcılığı bulunan gebe bir hastanın anestezi yönetimi

Abstract

Spinal musküler atrofi (SMA), progresif proksimal kas güçsüzlüğü ve atrofi ile karakterize, otozomal resesif geçişli bir nöromüsküler hastalıktır. Nöromüsküler hastalığı bulunan obstetrik hastalarda anestezi yönetimi, özellikle daha önce gelişmiş postdural ponksiyon baş ağrısı (PDPH) öyküsü ve eşlik eden solunumsal semptomların varlığında dikkatli bir planlama gerektirir. SMA taşıyıcılığı bulunan gebe bir hasta, tekrarlayan sezaryen öyküsü, gestasyonel hipertansiyon ve erken membran rüptürü nedeniyle sezaryen operasyonuna alınmıştır. Hastanın obstetrik öyküsünde 12. haftada missed abortus ve önceki sezaryen sırasında uygulanan spinal anestezi sonrasında gelişen PDPH yer almaktadır. Ayrıca hasta, beş gündür devam eden öksürük ve postnazal akıntı şikâyetleri ile başvurmuştur. Hastanın spinal anesteziyi reddetmesi ve genel anesteziye bağlı potansiyel riskler göz önünde bulundurularak, nöromüsküler blokör ajanlar ve trakeal entübasyondan kaçınılarak epidural anestezi uygulanmıştır. Epidural anestezi, SMA taşıyıcılığı bulunan ve PDPH öyküsü olan seçilmiş obstetrik hastalarda sezaryen doğum için güvenli ve etkili bir alternatif yöntem olabilir.
