Plak tip psoriazisde metotreksatın oral ve subkutanöz kullanımının kıyaslanması: 5 yıllık retrospektif inceleme

Öz

AMAÇ: Psoriazis, toplumun yaklaşık %2’sinde görülen, kronik, infl amatuar bir multisistem hastalığıdır. Metotreksatın oral formu psoriazis tedavisinde başarıyla kullanılmakla birlikte son yıllarda subkutanöz (SK) formu da geliştirilmiştir. Yakın zamanda bir araştırmada romatoid artritte SK metotreksatın aynı dozdaki oral metotreksata göre daha etkili olduğu gösterilmiştir. Bu çalışmada psoriazisde oral ile SK metotreksatın etkinlikve güvenilirlik açısından kıyaslanması amaçlanmıştır.YÖNTEMLER: Kliniğimizde 2006 ile 2010 yılları arasında metotreksat tedavisi almış olan plak tip psoriazisli hastaların kayıtları incelendi.BULGULAR: Çalışmaya 30 hasta (20 SK, 10 oral metotreksat grubunda) kaydedildi. Dördüncü haftada Psoriazis Alan Şiddet İndeksi (PAŞİ) 50 yanıtı SK metotreksat grubunda %50 iken, oral metotreksat grubunda %10’du (p=0.04). On altı hafta içinde PAŞİ 50 yanıtı sağlanana kadar alınan total metotreksat dozu oral metotreksat grubunda daha fazla olsa da (p=0.039), PAŞİ 75 yanıtı açısından total metotreksat dozu gruplar arasındafarklılık göstermedi (p>0.05). On ikinci ve 16. haftalarda bazale göre ortalama PAŞİ değişimleri gruplar arasında benzerlik göstermekteydi (p>0.05).SONUÇ: On ikinci ve 16. haftadaki ortalama PAŞİ değişimlerinin gruplar arasında benzer bulunması SK ve oralmetotreksatın etkinlik açısından birbirine üstünlükleri olmadığını göstermektedir. SK metotreksat ile dördüncühaftada daha fazla hastada PAŞİ 50 yanıtı sağlanmıştır. SK form indüksiyon tedavisi olarak kullanılabilir.

The comparison of oral versus subcutaneous administration of methotrexate in plaque type psoriasis: A 5 - year retrospective analysis

Abstract

OBJECTIVE: Psoriasis is a common, chronic, infl ammatory, multisystem disease affecting approximately 2% of thepopulation. While oral methotrexate has been used in the treatment of psoriasis with success, subcutaneous(SC) form of methotrexate have also been developed. SC methotrexate has been recently found more effectivethan oral methotrexate in the same dosage in rheumatoid arthritis. In this study, we aimed to compare theeffi cacy and safety of SC versus oral administration of methotrexate in patients with psoriasis.METHODS: We analyzed the records of the patients with plaque psoriasis under methotrexatetherapy at our clinic from 2006 to 2010.RESULTS: Thirty patients (20 SC and 10 oral) were enrolled. The ratio of Psoriasis Area Severity Index (PASI) 50 achievement at 4 weeks was 50% in SC group whereas only 10% in oral group (p=0.04). While the total methotrexate dosage taken until PASI 50 achievement within 16 weeks was greater in oral group (p=0.039), total methotrexate dosage did not differ between the groups in terms of PASI 75 achievement (p>0.05). Mean PASI changes from baseline were comparable at 12 and 16 weeks between the groups (p>0.05).CONCLUSION: That the mean PASI changes from baseline were comparable at 12 and 16 weeks between the groups shows SC and oral methotrexate have no superiority to each other. PASI 50 was achieved with SC methotrexate in more patients for 4 weeks. SC methotrexate can be used as an induction therapy.
