Anaesthetic management of emergency thoracic endovascular aneurysm operation in an aorta’s involved patient with neurovascular Behçet’s disease

Abstract

Behçet’s Disease is an autoimmune chronic multisystem inflamatory disorders, characterized by remissions and exacerbations. As a systemic disease, it can involve visceral organs such as gastrointestinal, pulmonary, musculoskeletal, cardiovascular and neurological systems. All anesthesia attempts from vessel canulation and intubation to anethesia medication may exacerbate the disease and precipitate organ failures in Behçet’s patients, so anaesthetic management in these patients should be carried cautiously both in elective and emergency operations. In this case report it was viewed the anaesthetic management of emergency endovascular stent grafting of thoracic aortic aneurysm, in a patient with neurovascular Behçet's Disease.

Aorta tutulumu olan nörovasküler behçetli hastada acil torasik endovasküler anevrizma ameliyatı sırasında anestezi yönetimi

Abstract

Behçet Hastalığı remisyon ve alevlenmeler ile seyreden otoimmün, kronik, multisistemik, inflamatuar bir hastalıktır. Sistemik bir hastalık olan Behçet Hastalığında gastrointestinal, solunum, kardiyovasküler, kas iskelet ve sinir sistemi tutulumu olabilir. Behçet hastalarında damaryolu kanülasyon ve entübasyon işleminden anestezik ilaç uygulamasına kadar olan tüm anestezik girişimler hastalığı alevlendirebilir ve organ toksitesine neden olabilir, bu yüzden hem elektif hem de acil ameliyatlarda anestezi yönetimi sırasında çok dikkatli olunmalıdır. Bu olgu sunumunda pulmoner artere bası yapan torasik anevrizması olan ve nörolojik tutulumu olan Behçet hastasında acil koşullarda torasik endovasküler anevrizma ameliyatı sırasındaki anestezi yönetimi gözden geçirildi.
