Radial arter greftinin büyük doğal adaptasyon yeteneği; tercih sebebi mi, değil mi?

Abstract

Koroner baypas ameliyatında akımı sağlamak için radial arterin kullanıldığı ilk uygulamalardan sonra,perioperatif spazm ve postoperatif erken tıkanma oranlarının yüksek bulunması nedeniyle bu uygulamalardanuzaklaşılmıştır. Ancak, uzun periodlardan sonra bile açık kalabilirliğinin yüksek olması sayesinde arter greftiseçenekleri arasında önemli yerini yeniden kazanmıştır. Baypas sırasında damar yatağı oluşturmak için kullanılanradial arterin gösterdiği anatomik ve akımsal adaptasyonun sergilendiği bir olguyu sunmayı amaçladık. Ayrıcaakım adaptasyonunun restenoz olarak yanlış algılanabileceğinin üzerinde durmayı amaçladık.

Abstract

Following the fi rst use of radial artery as a conduit for coronary bypass, it was abandoned because of higherrate of artery spasm during surgery and prematurely occlusion in postoperative period. However, by means ofits higher patency rates even after a long period, as a result of pedicled harvesting technique and pharmacologicmanipulation, radial artery regained being a remarkable choice of arterial grafting. Here we aimed to present acase in which a patient had been used radial artery for replacing to another vascular bed and its anatomic andfl ow adaptation after bypass surgery. Also we aimed to highlight the fact of fl ow adaptation of a radial artery graftwhich could be misdiagnosed as a restenosis.
