Yenidoğan bebeklerde direkt Coombs testi taraması ve pozitifliğinin morbidite üzerine etkisi; tek merkez deneyimi

Abstract

AMAÇ: Yenidoğan indirekt hiperbilirubinemi (İHB) etiyolojisinde immün hemolitik anemiler önemli bir yere sahiptir. Direkt Coombs Testi (DAT) immün kaynaklı hemolizi göstermede önemlidir. Eritrosite bağlı antikorlar coombs testi ile saptanır. Yenidoğan tüm bebeklerde rutin tarama yapılması tartışmalı olsa da bazı merkezlerde kord kanında kan grubu ve DAT taraması yapılmaktadır. Çalışmamızda retrospektif olarak hastanemizde doğan bebeklerin DAT sonuçlarını değerlendirmek, pozitiflik nedenlerini saptamak ve pozitifliğin morbidite üzerine etkilerini belirlemek amaçlanmıştır.YÖNTEMLER: Temmuz 2012-Nisan 2013 tarihleri arasında Keçiören Eğitim ve Araştırma Hastanesi Kadın Doğum Ünitesinde doğan tüm bebeklerin ve annelerinin dosyaları ve hastane otomasyon bilgileri geriye dönük olarak incelendi. Bebeklerin kord kan grubu ve DAT sonuçları, izlemde immün hemolitik hastalık geçirip geçirmediği, İHB nedeni ile hastaneye yatırılarak fototerapi alıp almadığı araştırıldı.BULGULAR: DAT toplam 1122 bebeğin %6,6’sında, İHB nedeniyle yatan bebeklerin ise %37,8’inde pozitif saptandı. Abstract

OBJECTIVE: Immune mediated hemolysis has an important role in the etiology of indirect hyperbilirubemia (IHB) in newborns. Immune mediated hemolysis can be shown using Direct Coombs Test (DAT). Antibodies bound to erythrocytes are determined by coombs test. DAT and blood group screening in cord blood are routinely performed in some centers, though controversial. In this study, our aim was to investigate the DAT positivity in newborns borned at our hospital and to evaluate the causes of DAT positivity and its effect on morbidity. METHODS: This study was performed at Kecioren Training and Research Hospital between July 2012 and April 2013. The electronic records of all newborns and their mothers were studied. Babies’ cord blood group and DAT results, presence of any immune hemolytic disease and a history of phototherapy due to IHB was investigated.
