PAS pozitif dermatofitozlarda klinik ve histopatolojik bulguların analizi

Abstract

AMAÇ: Genellikle dermatofitozlar rutin hayatta histopatolojik inceleme yapılmadan tanı almaktadır. Biyopside izlenen spongiotik veya psöriasiform dermatit bulguları dermatofitozlarda sık rastlanmakla birlikte, bu bulgular tanı için yeterli değildir. Çalışmamızın amacı dermatofitoz tanısına yardımcı olabilecek spesifik histopatolojik bulguları araştırmak ve Periyodik asit-Schiff [PAS] histokimyasal yönteminin tanısal değerini ortaya koymaktır. YÖNTEMLER: Bu çalışmada hematoksilen ve eozin [H&E], PAS boyama teknikleri ile dermatofitoz tanısı alan 19 hastanın boyalı preparatları ve klinik verileri tekrar değerlendirildi. Olguların klinik özellikleri, histomorfolojik ve histokimyasal bulguları, ön tanıları ile karşılaştırıldı. BULGULAR: Histopatolojik ve PAS inceleme ile tanı alan olgularımızın %68.4’ünde, klinisyenin ön tanıları arasında fungal dermatit tanısı yoktu. PAS histokimyasal boyası ile endotriks tanısı alan biyopsi örneklerinin H&E boyalı kesitlerinde saptanan perifolikülit bulgusu istatistiksel olarak anlamlıydı. Ayrıca eşlik eden parakeratoz bulgusunun sıklığı da anlamlı bulundu. SONUÇ: Spongiotik veya psoriasiform dermatitlerde mantar ön tanısı olmasa da, deri biyopsilerinde rutin histokimyasal PAS boyaması mantar tanısında gereklidir. Ayrıca dermatofitozların bilinen tipik histopatolojik bulgularına ek olarak, parakeratoz ve endotriks varlığı ile birlikte perifolikülit bulgularının da tanıya yardımcı bulgular olabileceği akılda tutulmalıdır.

Abstract

OBJECTIVE: In most cases, dermatophytosis is diagnosed routinely without histopathological examination. Spongiotic or psoriasiform features are frequently found in fungal dermatoses but these histopathological findings are not proficient for diagnosis. By the way, the aim of the study is to identify characteristic histopathogical clues and to emphasize the diagnostic significance of dermatophytosis stained with histochemical method of Periodic acid-Schiff [PAS]. METHODS: In this study; PAS and Hematoxylin and eosin [H&E] stained slides and their clinical parameters of 19 dermatophytose cases confirmed by PAS stain were reevaluated. We compared histopathological and histochemical findings with clinical findings of all cases.RESULTS: Fungal microorganisms were identified with PAS stained slides in %68.4 cases in which fungal infection had not been suspected clinically. We detected perifolliculitis in all endothrix infection visualised cases. This finding and the presence of parakeratosis were significant for fungal infections. CONCLUSION: Therefore, PAS stain should be considered on any spongiotic or psoriasiform dermatitis, and perifolliculitis, parakeratosis should be kept in mind as helpful pathologic clues to facilitate an accurate diagnosis of dermatophytic infections.
