Obez çocuk ve adolesanlarda vücut kitle indeksi ve biyoelektrik impedans yöntemi ile ölçülen vücut yağ oranının lipid düzeyleri ve metabolik parametrelerle ilişkisi

Abstract

Amaç:
 Bu çalışma ile obez çocuklarda
biyoelektrik impedans yöntemi ile değerlendirilen vücut yağ oranının (VYO), vücut
kitle indeksi standart sapma (VKİ-SDS) ve bel kalça oranına (BKO) kıyasla istenmeyen
lipid profili ve metabolik parametreleri öngörmede etkinliğinin
değerlendirilmesi amaçlanmıştır.

Gereç
ve Yöntem: Bu çalışmaya 57 obez, 35 sağlıklı çocuk dahil edilmiş olup,  boy standart sapma skoru (HSDS), kilo standart
sapma skoru (WSDS), VKİ-SDS, BKO hesaplanmıştır. Biyoelektrik impedans yöntemi
ile VYO saptanmıştır. Hastaların kan basınçları,glikoz, insülin, total
kolesterol (TK), trigliserid (TG), yüksek dansiteli lipoprotein (HDL), düşük
dansiteli lipoprotein (LDL) ve homeostatik model değerlendirme insülin direnci
(HOMA-IR)  seviyeleri değerlendirilmiştir.

Bulgular:
VKİ-SDS, BKO, VYO, sistolik ve diyastolik kan basıncı, TG, TK, LDL, insülin ve
HOMA-IR değerleri obez grupta daha yüksek olarak bulundu (p<0.05). HDL ve glikoz
değerleri yönünden gruplar arasında fark saptanmamıştır (p>0.05). Pearson
korelasyon analizinde VKİ-SDS ve VYO ile BKO, sistolik ve diastolik kan
basıncı, insülin, HOMA-IR, TK, TG, HDL, LDL arasında pozitif ilişki varken, HDL
ve glikoz arasında ilişki gözlenmemiştir.







Sonuç:
Metabolik bozukluk ve dislipidemiyi öngörmek açısından  BİA ile tespit edilen VYO’ nın, kilo
belirteçlerinden VKİ ve BKO’ ya göre daha üstün olmadığı, VKİ-SDS’ nin ise BKO
‘na göre lipid bozukluğunu öngörme açısından daha iyi olduğu saptandı . VKI ve
VYO ‘nun beraber kullanımının ise obeziteye bağlı gelişen hipertansiyon,
metabolik bozukluk ve kardiyovasküler komplikasyonları önlemek açısından çok
daha önemli olduğu sonucuna varıldı.
