Sudden Silent Deaths

Abstract

Sudden cardiac death (SCD) is a rapidly fatal event most commonly caused by coronary artery disease, structural heart disorders, or primary electrical abnormalities. Electrical diseases predominate in younger individuals, while hypertrophic cardiomyopathy and inherited arrhythmia syndromes are major contributors in athletes. In dialysis patients, low-potassium dialysate, rapid ultrafiltration, and the first post-weekend session significantly increase arrhythmic risk. In heart failure, arrhythmic deaths dominate in mid-stage disease, whereas pump failure becomes prominent in advanced stages. Early identification of risk factors and implementation of preventive strategies are essential to reduce SCD-related mortality.

Abstract

Ani kardiyak ölüm (AKÖ), çoğunlukla koroner arter hastalığı, yapısal kalp hastalıkları veya primer elektriksel bozukluklar sonucu ortaya çıkan, dakikalar içinde ölümle sonuçlanabilen kritik bir tablodur. Genç bireylerde elektriksel hastalıklar daha belirginken, sporcularda HKMP ve kalıtsal aritmi sendromları önemli nedenlerdir. Diyaliz hastalarında düşük potasyumlu diyalizat, hızlı ultrafiltrasyon ve hafta sonu aralığı sonrası yapılan ilk seans aritmi riskini artırır. Kalp yetersizliğinde orta evrede aritmik ölümler baskınken, ileri evrede pompa yetmezliği ön plana çıkar. Risk faktörlerinin erken tanınması ve korunma stratejilerinin uygulanması AKÖ’ye bağlı mortaliteyi azaltmada temel yaklaşımdır.
