Akut Çölyak Krizi Olgusu: Yetişkinlerde Nadir Ancak Yaşamı Tehdit Eden Bir Komplikasyon

Abstract

Çölyak hastalığı, genetik yatkınlığı olan bireylerde gluten tüketimiyle tetiklenen otoimmün bir enteropatidir. Genellikle gastrointestinal semptomlarla seyretse de multisistemik bulgularla da prezente olabilir. Nadir görülen ve yaşamı tehdit edebilen bir komplikasyon olan çölyak krizi ise, metabolik bozukluklar, elektrolit dengesizlikleri, dehidratasyon, renal fonksiyon bozukluğu ve nörolojik semptomlarla karakterizedir. Bu sunumda akut çölyak krizi ile acil servise başvuran yetişkin bir çölyak olgusu sunulmuştur. Bu nadir olgu sunumu ile çölyak hastalığının mortalitesi yüksek prezantasyonuna dikkat çekilmesi amaçlanmıştır. Çölyak krizi, patogenezinde şiddetli mukozal inflamasyona bağlı gelişen malabsorpsiyon, protein ve elektrolit kayıpları temel rol oynamaktadır. Kriz; enfeksiyon, cerrahi girişimler veya diyet uyumsuzluğu gibi tetikleyici faktörlerle ortaya çıkabilir. Bu olgu sunumu ile çölyak hastalığının sadece klasik gastrointestinal belirtilerle değil, sistemik ve atipik akut bulgularla da prezente olabileceğine dikkat çekilmektedir. Özellikle dirençli ishal, elektrolit bozuklukları ve dehidratasyon ile başvuran erişkin hastalarda ayırıcı tanıda çölyak krizinin göz önünde bulundurulması gerektiği vurgulanmaktadır. Erken tanı ve uygun tedavi, mortaliteyi azaltmada kritik öneme sahiptir.

Acute Celiac Crisis: A Rare but Life-Threatening Complication in Adults

Abstract

Celiac disease is an autoimmune enteropathy triggered by gluten ingestion in genetically predisposed individuals. While typically characterized by gastrointestinal symptoms, its clinical spectrum can include extraintestinal manifestations. Celiac crisis, a rare and life-threatening complication, is distinguished by severe metabolic derangements, profound electrolyte imbalances, dehydration, and renal dysfunction. Herein, we report the case of an adult diagnosed with celiac disease who was admitted to the emergency department in a state of acute celiac crisis. This report aims to highlight a presentation of celiac disease associated with significant mortality. The pathogenesis of celiac crisis is primarily driven by severe mucosal inflammation, leading to extensive malabsorption and consequent protein and electrolyte losses. The crisis can also be caused by precipitating events such as infections, surgical stress, or a lack of adherence to a gluten-free diet. This case illustrates that celiac disease can be acutely manifest with systemic and unusual presentations alongside its classical gastrointestinal presentation. Thus, celiac crisis must be among the first differential diagnoses in refractory diarrhea with serious metabolic disturbances in adult patients. Early detection and aggressive treatment are important for reducing the mortality rate of the condition.
