İntestinal obstrüksiyonlar ve herniler bağırsak hastalıklarında en sık cerrahi girişim gerektiren ve hastanın yaşam kalitesini etkileyen hastalıklardır. Pediatri hemşireliğinde verilecek bakımının temelinde; çocuğun sıvı-elektrolit dengesinin sürdürülmesi, ağrının kontrol edilmesi, düzenli ve dengeli beslenmesinin sağlanması, olası komplikasyonların (kanama, perforasyon, enfeksiyon riski vb.) önlenmesi-erken tanılanması-uygun tedavi ve bakımın sağlanması yer almaktadır. Çalışmamızın amacı intestinal herni ve septisemi tıbbi tanıları ile çocuk yoğun bakım ünitesinde izlenen bir çocuk hastanın Roper-Logan-Tierney Hemşirelik Modeline (RLTHM) göre hemşirelik bakımının planlanması ve uygulanmasıdır. Sonuç: Roper-Logan-Tierney Hemşirelik Modelinde (RLTHM) yer alan 12 günlük yaşam aktivitesine uygun olarak toplamda 14 hemşirelik tanısı tespit edilmiştir. Hemşirelik bakımında model kullanımının yaygınlaşması hastanın daha iyi değerlendirilmesi ve bakımın kalitesi açısından önemlidir.

Intestinal obstructions and hernias are the diseases that most frequently require surgical intervention in intestinal diseases and affect the patient's quality of life. Maintaining the child's fluid-electrolyte balance, controlling pain, ensuring a regular and balanced diet, preventing possible complications (bleeding, perforation, risk of infection, etc.)-early diagnosis-providing appropriate treatment and care are the interventions of pediatric nursing for both health problems. Our case report aimd to plan and implement nursing care according to the Roper-Logan-Tierney Nursing Model (RLTHM) of a pediatric patient followed in the pediatric intensive care unit with medical diagnoses of intestinal hernia and septicemia.Conclusion: A total of 14 nursing diagnoses were determined in accordance with the 12 activities of daily living in the Roper-Logan-Tierney Nursing Model (RLTHM). The widespread use of models in nursing care is important in terms of better evaluation of the patient and the quality of care.
