Niemann-Pick Hastalığı İle Birlikte Gezici Dalak Olgusu

Abstract

Gezici dalak, dalağı normal pozisyonunda tutan asıcı bağlarının doğuştan yokluğu veya akkiz gevşekliği sonucu dalağın hareketli hale gelmesi ve karnın alt bölgelerinde ya da pelviste yerleşmesidir. Doğurganlık çağındaki kadınlarda sıktır, çocuklarda nadiren görülür. Hastada, torsiyonu sonucu akut ya da kronik tekrarlayan karın ağrıları oluşabilir. Asemptomatik olup başka bir sebeple yapılan muayene ya da tetkik sonucunda da ortaya çıkabilir. Niemann-Pick hastalığı bir lipid depo hastalığıdır. Öncelikle karaciğer ve dalakta olmak üzere pek çok organda sfingomiyelin ve kolesterol birikmesi ile karakterizedir. Hepatosplenomegali gelişebilir. Niemann-Pick hastalığı nedeniyle takip edilen 16 yaşında kız hasta adet görememe nedeniyle tetkik edilirken sol over kökenli olabilecek 10 cm boyutunda pelvik kitle tespit edildi. Eksploratif laparotomide karın sol altta 12 cmlik gezici dalak ve hilusunda soluk renkte dalak ile füzyon yapmış, düzensiz yapıda, 5 cm boyutunda solid bir kitle tespit edildi, hastaya splenektomi yapıldı. Patolojisi dalakda ceroid histiositozis ve hilusundaki kitlede nekroz ve sklerozis olarak rapor edildi. Niemann-Pick hastalığında splenomegali gelişmesine ve splenomegalinin gezici dalak için etiyolojik faktörler arasında sayılmasına rağmen literatürde daha önce bu iki hastalığın birlikteliği bildirilmemiştir. Asemptomatik olan gezici dalak olguları klinikte karında kitle ile özellikle de hastamızda olduğu gibi overian kitle ile karışabilmektedir. Bu nedenle bu nadir birliktelik bu olgu sunumunda tartışılmıştır.

Wandering Spleen with Niemann-Pick Disease

Abstract

Wandering spleen is a rare entity that develops as a result of splenic hypermobility caused by congenital absence or acquired laxity of splenic suspensory ligaments that hold the spleen in its normal position. It is usually described in women of childbearing age and rare in children. Symptoms include acute or chronic intermittent abdominal pain due to the torsion of the pedicle. It may also be asymptomatic and discovered incidentally on clinical or radiological examination done for other reasons.Niemann-Pick disease is a lipid storage disorder. It is characterized by sphingomyelin and cholesterol accumulation in many organs and predominantly the liver and spleen. Hepatosplenomegaly may develop due to the disease. A 16-year-old girl with Niemann-Pick disease has admitted to our hospital with complaint of amenorrhea. A 10 cm pelvic mass possibly related to the left ovary was detected in her examinations. Laparotomy was performed and a 12 cm wandering spleen was discovered at the left lower quadrant of abdomen. And 5 cm faint, solid mass was found to be fused with the spleen at hilus. Splenectomy was performed. Histopathological examination revealed ceroid histiocytosis throughout spleen. The hilar mass was found to be a necrotic and sclerotic tissue.Although Niemann-Pick disease is reported as an etiological factor for splenomegaly as a cause of wandering spleen in the literature, their association is not reported yet. Asymptomatic wandering spleen can be regarded as abdominal or ovarian mass as in our case. We discussed this case to point out the differential diagnosis of wandering spleen and its rare association with Niemann-Pick Disease

Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication, with at least 10 original articles in each issue, taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.
