Medicolegal importance of pathological findings in metabolic diseases of the central nervous system

Abstract

Introduction: Some metabolic diseases of the central nervous system can lead to descriptive morphological changes. Therefore pathologies of the central nervous system related to chronic alcohol consumption and tiamin deficiency have called the researchers' attention for many years. The aim of our study is to describe the clinical and morphological characteristics of these pathologies, and to emphasize their importance within the medico-legal practice. Results: Acute Wernicke encephalopathy and Korsakoff psychosis, which reflects the chronic process, are two of the most frequently studied diseases related with chronic alcohol consumption and tiamin deficiency. Cerebellar atrophy and central pontin myelinosis, which accompanies severe electrolyte disorders, are also to be considered in this group. And although seen infrequently, Marchiafava-Bignami disease, a disease appearing in cheap red wine drinkers, is studied in this group as well. Conclusion: Knowledge about such pathologies, which are rarely seen in surgical pathology but with strong effects on the central nervous system, and their accurate interpretation is very important in legal autopsy applications

Merkez sinir sistemi metabolik hastalıklarındaki patolojik bulguların adli tıp açısından önemi

Öz

Giriş: Santral sinir sisteminin bazı metabolik hastalıkları, tanımlayıcı olabilen morfolojik değişiklikler oluştururlar. Kronik alkol kullanımı ve tiamin eksikliğine bağlı gelişen santral sinir sistemi patolojileri de uzun yıllardır araştırmacıların dikkatini çekmiştir. Çalışmada amacımız, bu patolojilerin klinik ve morfolojik özelliklerini tanımlayıp, bu bulguların adli tıp pratiği içinde önemini vurgulamaktır. Bulgular: Kronik alkol kullanımı ve tiamin eksikliğine bağlı gelişen hastalıklar içerisinde bugün en çok çalışmaya konu olanlardan biri Akut Wernicke ensefalopatisi ve kronik süreci yansıtan Korsakoff psikozu’dur. Bunun dışında, serebellar atrofi ve ağır elektrolit bozukluklarına eşlik eden Santral pontin myelinolizis de bu grupta yer almaktadır. Ayrıca, seyrek görülmekle birlikte, ucuz kırmızı şarap içicilerinde rastlanabilen Marchiafava-Bignami hastalığı da aynı grup hastalıklar içinde incelenmiştir. Sonuç: Cerrahi patolojide sık rastanmayan, öncelikle santral sinir sistemine olan etkileri güçlü olan bu tür patolojilerin bilinmesi ve doğru yorumlanması adli otopsi pratiği açısından büyük önem taşımaktadır
