Ganglioglioma of the brainstem: A case report

Abstract

Purpose: Gangliogliomas are unusual tumors of CNS comprised of variable proportions of ganglion cells and glial cells. Both cellular groups show cytologic features of neoplasia. Gangliogliomas represent less than %2 of intracranial neoplasms. These tumors are more common in children and they are often found in the temporal lobe. The purpose of our study, is to evaluate autopsy findings, histopathologic and immunohistochemical features of a sudden death due to localization of this tumor. Case report: A well circumscribed tumor with brainstem localization found in the autopsy of a 4 year old boy, followed up for slep apnea, hospitalized with disphagia and aspiration pneumonia, and died suddenly after seven days during EEG is discussed concerning the macroscopic, histopathologic and immunohistochemical features. Beyin sapı yerleşimli gangliogliomu: Olgu sunumu

Öz

Amaç: Gangliogliomlar, ganglion hücreleri ve glial hücrelerin değişen oranlarda bir araya geldiği nadir rastlanan tümörlerdir. Bu tümörlerde her iki hücre grubu da neoplastik değişim özellikleri gösterebilir. Gangliogliomlar, intrakranyal tümörlerin %2’sinden azını oluşturmaktadır. Bu tümörler çocukluk çağında daha sık görülmekte olup, sıklıkla temporal lob yerleşimi göstermektedir. Amacımız, yerleşimi nedeni ile ani ölümle sonuçlanan olguya ait otopsi bulguları, histopatolojik ve immun doku kimyasal özelliklerin kaynaklar ışığında değerlendirilmesidir. Olgu Sunumu: Uyku apneleri nedeni ile takip edilen, disfaji, aspirasyon pnömonisi tabloları ile interne edilen ve 7 gün sonra EEG çekimi sırasında aniden ölen, 4 yaşında erkek hastanın otopsisinde saptanan iyi sınırlı, beyin sapı yerleşimli tümör, makroskopik, histopatolojik ve immun doku kimyasal özellikleri ile değerlendirilmiştir. Mikroskopik değerlendirmede, tümörün astrositler ve nöronlardan oluşan mikst hücre üreyişi ile karakterize olduğu izlendi. Tümördeki nöral elemanlarda sinaptofizin, astrositik komponentte GFAP ile sitoplazmik boyanma saptandı. Astrositik komponentte Ki-67 indeksi %2,1 reaction with synaphtophysin, and astrocytic component olarak saptandı. Bulguların ışığında olgu, beyin sapı showed cytoplasmic reaction with GFAP. Ki-67 index is yerleşimi gösteren Grade II Gangliogliom olarak değerlendirildi. Sonuç: Gangliogliomlar, düşük dereceli ve uzun klinik öyküye sahip tümörlerdir. Bununla birlikte, ender olarak otopsi uygulamalarında ani ölüm şeklinde karşımıza çıkabilmektedir
