Bilateral multiloculated cystic renal cell carcinoma (Case report)

Abstract

Aim: Multiloculated cystic renal cell carcinoma is a rare variant of renal cell carcinoma. Incidence and biological behaviour of the tumor are unknown and bilateral cases are very rare. Case report: Fifty four-years- old male patient was admitted to the Urology policlinic with a left flank pain which was present during one month. On ultra sonographic examination solid hypoecoic mass 37x 32 mm in size and extending to the adrenal area were found at the upperpole of right kidney. Another mass 38x34 mm in size, with mixt nature and smooth contours and finely vascular flow were detected in middle-upper pole of the left kidney. The patient had a urinary stone history. On CT scan; each kidney were normal in size and a cystic lesion 3.5 cm in diameter and with thickened wall was located on cortical-parapelvic region of the upper half of the left kidney. Exophytic elongated cystic lesion was also observed on right kidney upper pole which had a thick superolateral wall. Conclusion: Right radical and left partial nephrectomy were performed. On macroscopic examination; lesions with many cysts were detected in both kidneys which were diagnosed histopathologically as multiloculated cystic renal cell carcinoma

Bilateral multiloküle kistik renal hücreli karsinom (Olgu sunumu)

Öz

Amaç: Multiloküle kistik renal hücreli karsinom (MKRHK), renal hücreli karsinomun az görülen bir tipidir. Bilateral olguların çok daha az olduğu bildirilmektedir. Olgu Sunumu: Elli-dört yaşında erkek hasta, 1 aydır süren sol yan ağrısı nedeniyle Üroloji polikliniğine başvurmuştur. İki kez taş düşürme öyküsü olan hastaya yapılan USG’de; sağ böbrek üst polde adrenal lojuna uzanan 37x32 mm boyutlarda solid hipoekoik bir kitle, sol böbrek orta-üst polde de 38x34 mm boyutlarda, mikst natürde, düzgün konturlu, ince vasküler akımlı bir başka kitle saptandı. BT’de; her iki böbrek boyutlarının normal olduğu, sol böbrek üst yarıda kortikal-parapelvik yerleşimli yaklaşık 3.5 cm çapında, duvarında kalınlaşmalar izlenen kistik lezyon, sağ böbrek üst polde ise ekzofitik uzanımlı, superolateral duvarı kalın, kortikal yerleşimli kistik lezyon izlendi. Sonuç: Olguya sağ radikal ve sol parsiyel nefrektomi yapıldı. Makroskopik incelemede; her iki böbrekte çok sayıda kistler içeren lezyonlar saptandı. Histopatolojik olarak multiloküle kistik renal hücreli karsinom tanısı aldı
