Wegener’s granulomatosis of the upper aerodigestive tract: A case report

Abstract

Introduction/Purpose: Wegener’s granulomatosis is a chronic and systemic disease. The etiology of disease is still unknown. It is defined as a granulomatous inflammation of the upper and lower respiratory tract and systemic vasculitis of small and medium sized vessels often accompanied by a necrotizing glomerulonephritis. The initial diagnosis of Wegener’s granulomatosis is based on clinical features, positive cytoplasmicantineutrophil cytoplasmic antibody test and histological findings. However, the negative antineutrophil cytoplasmic antibody test does not exclude Wegener’s granulomatosis. In this article we present a case of Wegener’s granulomatosis with histopathologic findings. Case report: A 54 year-old female patient was admitted to the hospital with complaints of a pain in her nose, hemorhage and obstruction. The biopsises were taken from the both choncae. There was a chronic inflammation including neutrophils, plasma cells, lymp hocytes, histiocytes and eosinophils in the stroma that formed microabscesses and granulomas. Fibrinoid necrosis, fibrosis and obliteration were seen in the vessel walls. The case was diagnosed as a Wegener’s granulomatosis with its Sonuç: Wegener granulomatozu oldukça az görülen sistemik bir hastalıktır. Rinolojik semptomlar, olguların en sık doktora başvuru nedenidir. Hastalığın erken tanı ve sağaltımı prognozu etkilediği için kuşkulu olgular dikkatle gözden geçirilmelidir. Bu çalışmada, diğer granülomatoz enflamasyon nedenleri ile Wegener granulomatozunun ayırımı klinik ve histopatolojik bulgular ışığında değerlendirilmiştir. Conclusion: Wegener’s granulomatosis is a rarely seen systemic disorder. Patients often complain of rhinologic symptoms. Because early diagnosis and treatment has an effect on the prognosis, the suspected patients should be examined carefully. In this article the features of Wegener’s granulomatosis have been presented with differential diagnosis of other granulomatous diseases

Üst solunum yollarında Wegener granulomatozu: Olgu sunumu

Öz

Giriş/Amaç: Wegener granulomatozu, kronik sistemik bir hastalıktır. Etyolojisi henüz bilinmemektedir. Üst ve alt solunum yollarının granülomatöz yangısı ile küçük ve orta boyutlu damarların sistemik vaskülitine eşlik eden nekrotizan glomerulonefritle karakterizedir. Tanı, klinik bulgular, sitoplazmik antinötrofil sitoplazmik antikor (cANCA) pozitifliği ve histolojik bulgular zemininde konur. Ancak negatif c-ANCA testi Wegener Granulomatozu tanısını ekarte ettirmez. Bu makalede üst solunum yollarını tutan bir Wegener granulomatozu olgusu histopatolojik bulguları ile sunulmuştur. Olgu sunumu: 54 yaşında kadın hasta, 1 aydır devam eden burunda ağrı, akıntı, sızıntı şeklinde burun kanaması ve burun tıkanıklığı yakınmaları ile başvurdu. Her iki konkadan biopsiler alındı. Histopatolojik incelemede parankimde, nötrofillerden zengin, plazma hücreleri, lenfositler, histiyositler ve eozinofilleri içeren mikroabseler yanı sıra damar duvarlarında akut ve kronik yangısal hücre infiltrasyonu ile dev hücreler görüldü. Granulomatoz inflamasyon, mikroabselere eşlik ediyordu. Damar duvarlarında fibrinoid nekroz, konsantrik fibrozis ve bazı alanlarda damar lümenlerinde fibröz obliterasyon izlendi. Olgumuza klinik ve histolojik bulgular yanısıra pozitif clinical and histologic findings also with the positivity of c-ANCA testi ile Wegener granulomatozu tanısı kondu. c-ANCA test
