Ulusal Bir Dış Kalite Kontrol Programına Olan İhtiyaç Ve Ülkemiz Şartları

Öz

Tıbbi laboratuvarlar her sağlık sisteminde bilgi akışının tam merkezinde yer alırlar. Klinisyenler tanı ve tedavilerini rutinde yaptıkları analizlerin sonuçlarına dayandırırlar. Bu analizler klinik kararları %75 oranında etkilerler. İdeal şartlarda, nerede yapıldığına bağlı olmaksızın, test sonuçlarının tüm ülkede tutarlı, doğru ve güvenilir olmasını ulusal düzeyde sağlayan ve bunu izleyen bir sisteme sahip olunması arzu edilmektedir. Diğer ülkelerden elde edilen deneyimler herhangi bir düzene tabi olmayan laboratuvarların, tipik olarak kendilerince birtakım prosedürler/protokoller uyguladıklarını ve ürettikleri test sonuçlarının da sıklıkla standartların altında ve yanlış olduğunu ortaya koymaktadır. Bu şartlar altında test protokolleri laboratuvardan laboratuvara önemli derecede değişmektedir. Bu laboratuvarlar yakından incelendiğinde de ISO 9000-2000; ISO 15189 gibi uluslararası tanınmış süreç yönetimi standartlarına ve tıbben uygun analitik performans standartlarına göre belirgin bir şekilde eksik oldukları görülmektedir. Ulusal ve/veya uluslararası yeterlilik testleri ve laboratuvarlar arası karşılaştırma; klinik laboratuvarların kalite kontrolünün en önemli bileşenlerinden olup akredite olmayı hedefleyen laboratuvarlar için temel kalite araçlarındandır. Türkiye’deki klinik laboratuvarlar 992 No’lu Kanun olarak bilinen 1927’de yasalaşmış mevzuata göre değerlendirilmektedir. Ülkemizdeki sağlık sisteminde tıbbi laboratuvarlar tarafından sağlanan analizlerin kalitesini izleyen ve güvence altına alan ulusal bir sistem bulunmamaktadır. Bu derlemede Türkiye’de ulusal bir dış kalite kontrol programı kurulmasının gerekliliği ve Refik Saydam Hıfzıssıhha Merkezi Başkanlığı’nın bu oluşuma adres olup olamayacağı tartışılmıştır

Needs for National External Quality Program and Conditions of the Country

Abstract

Medical laboratories are at the hub of the information flow within any health care system. Physicians make diagnosis and management depend on the test results generated routinely. These analyses trigger clinical decisions at a rate of 75%. At ideal conditions, it would be desirable to have a system that monitors and ensures at a national level that all test results within a country are accurate, consistent and reliable regardless of wherever analyses are performed. Experience from other countries has revealed that unregulated medical laboratories typically operate with a series of in-house procedures/protocols and the test results they produce are often sub-standard and inaccurate. Under such circumstances, the testing protocols vary significantly from laboratory to laboratory When these laboratories were investigated closely, it can be seen that they are noticeably deficient if they compared to internationally recognized process management standards such as ISO 9000-2000; ISO 15189 and to medically relevant analytical performance standards. Proficiency Tests PT and Inter-Laboratory Comparison ILC are the most important components of quality control in clinical laboratories. National and/or international PT and ILC programs are the major tools for the laboratories aiming to be accredited. The clinical laboratories in Turkey are assessed under the legislation accepted by 1927 which is known the law 992. The health care system in our country does not have a national system for monitoring and ensuring the quality of analyses that is being provided by the medical laboratories. In this review, it was weighed weather a national external quality control program is needed in Turkey and Refik Saydam National Public Health Agency would be the right address for this program to be set up.
