GATA Haydarpaşa Eğitim Hastanesi’ndeki sağlık çalışanlarının pandemik influenza A/H1N1 aşılaması ve aşıya bağlı yan etkiler

Öz

Amaç: Dünya Sağlık Örgütü, 2009 yılında ortaya çıkan influenza A H1N1 pandemisi ile mücadelede, sağlık çalışanlarının aşılanmasının birinci öncelikli hedef olması gerektiğini bildirmiştir. Kısa ve uzun vadede aşı yan etkileri konusundaki belirsizlik tartışma konusu olmuş ve aşıyı kabul etmeyi etkileyeceği öne sürülmüştür. Bu çalışmada sağlık çalışanları arasında pandemik influenza A/H1 N1 aşılamasını kabul etme, aşı ile ilişkili yan etkiler ve bu etkilerin görülme sıklığının belirlenmesi amaçlanmıştır.Yöntem: 20 Kasım-01Aralık 2009 tarihleri arasında GATA Haydarpaşa Eğitim Hastanesinde sağlık çalışanları arasında pandemik influenza aşı uygulamasını kabul etme ve aşı ile ilişkili yan etkiler prospektif gözlemsel olarak araştırılmıştır. Gönüllü olarak aşı uygulanan personel üç grupta sınıflandırılmıştır. Her grup için aşılanan personel ile ilgili klinik bilgiler ve aşı uygulaması sonrası saptanan yan etkiler hazırlanan formlara kayıt edilmiştir

Pandemic influenza A/H1N1 vaccination and vaccine-related side effects in health care workers of GATA Haydarpasa Training Hospital

Abstract

Objective: Vaccination of health care workers was declared as the first priority by WHO in encountering with H1N1 influenza virus, which caused a worldwide pandemic in 2009. However, lack of knowledge of long and short term side effects of the vaccine could effect the acceptibility of the vaccination among health care workers. In this study, it was aimed to determine the acceptibility of influenza A/H1N1 vaccination, and the incidence of vaccine-related side effects among health care workers.Method: A prospective observational study have been conducted to examine the acceptance and side effects of pandemic influenza vaccine among health care workers at GATA Haydarpasa Training Hospital, between November 20 and December 1, 2009. Voluntarily vaccinated personnel were assigned into three groups. and the clinical data of the vaccinated personnel, as well as side effects after vaccination were recorded.Bulgular: Toplam 1.185 sağlık personeli çalışanımız arasından gönüllü olan 669’una pandemik influenza A H1N1 aşısı uygulanmıştır. Aşılanan personelin aşılamayı kabul etme yüzdeleri incelendiğinde doktor grubunda oranın %84,3 291/345 , hemşirelerde %52,9 198/374 , yardımcı sağlık personelinde ise %38,6 180/466 olduğu saptanmıştır. Aşılama sonrası 14 gün aktif, sonraki bir yıl içerisinde bildirime dayalı pasif sürveyans ile personel takip edilmiştir. Toplam 261 %38,6 aşı ile ilişkili yan etki belirlenmiştir. Aşılanan personelin 62 %9 ’si sadece lokal, 89 %13,3 ’u ise lokal ve sistemik yan etkileri birlikte bildirmiştir. En sık yan etkisi olarak aşı uygulanan bölgede ağrı %17,3 , halsizlik %7,6 ve baş ağrısı %6,7 saptanmıştır. Aşılanan personelin hiçbirisinde hayatı tehdit eden ciddi yan etki belirlenmemiştir.Sonuç: Halk sağlığını tehdit eden durumlarda, özellikle sağlık hizmetlerinde görevli personelin daha iyi bilgilendirilmesi ve eğitimi ile önyargı oluşmasını önleyerek aşıya uyumu sağlanması salgın kontrolünde en önemli adımlardan birisidir.Results: Out of 1,185 health care workers, 669 56.5% were vaccinated voluntarily with pandemic influenza A /H1N1 vaccine. The percentage of acceptance was 84.3% 291/345 for doctors, 52.9% 198/374 for nurses and 38.6% 180/466 for other health personnel. Health care workers were followed actively for 14 days after vaccination and a passive surveillance have been conducted based on the staff notices within one year. In total, 261 38.6% side effects were reported related to vaccination; 62 9% of those were only local, while 89 13.3% were both systemic and local. The most frequently seen side effects were pain on the injection site 17.3% , dizziness 7.6% and headache 6.7% . Lifethreatening side effects were not observed in any of the vaccinated personnel.Conclusion: Training of health care workers is one of the most important step for an effective vaccination campaign of public health importance. This could prevent prejudices and ensure compliance with the vaccination while controlling the epidemic
