İmmun sağlam fakat yoğun emosyonel stres altında olan iki olguda Zona zoster sonrası gelişen aseptik menenjit ve hepatit tablosu

Abstract

Zona zoster, immün sistemi sağlam olanlarda sıklıkla iyi seyirli bir hastalıktır. İmmun sistemi baskılanmış hastalarda ise yaygın olma riski taşır. Zona zoster, hem immun sistemi sağlam, hem de zayıflamış hastalarda nadiren miyelit, aseptik menenjit, ensefalit ve ventrikulit gibi nörolojik komplikasyonlara yol açabilir. Yirmi ve yirmi bir yaşında immunsistemleri sağlam olan iki erkek hasta ortak olarak ateş, şiddetli baş ağrısı, uyku hali, bulantı, kusma yakınmaları ile hastanemiz acil servisine farklı zamanlarda başvurdular. Ek olarak bir hastada yürüme ve denge problemi şikayeti vardı. Hastaların yapılan fizik muayenelerinde bir olguda sol koltuk altında diğer olguda sol lumbal bölgede belli bir dermatom alanı ile sınırlı gruplar halinde vezikuler lezyonlar saptandı. Yine iki olguda da ense sertliği ve meninks irritasyon bulguları pozitifti. Her iki olguya da lomber ponksiyon yapıldı. Beyin omurilik sıvısında lenfosit hakimiyetinde pleositoz ve protein yüksekliği görüldü. Epidemiyolojik olarak birbirleriyle ilişkisi olmayan ve farklı zamanlarda başvuran hastalar servisimize yatırıldı. Alınan anamnezlerinde hastaların her ikisinin de son zamanlarda yoğun emosyonel stres altında oldukları anlaşıldı. Fizik incelemelerinde iki olguda da tek dermatom alanı ile sınırlı döküntüler tespit edildi. Hastalara klinik ve laboratuar olarak aseptik menenjit ile komplike zona zoster tanısı konuldu. Beyin omurilik sıvısında VZV-PCR çalışılmadı ancak klinik ve her iki olgunun da serum VZV IgM serolojilerinin pozitif olması üzerine hastalarda aseptik menenjit ile komplike zona zoster tanısı konuldu. Her iki olguya da asiklovir tedavisi başlandı. Komplikasyonuz olarak iyileşme sağlandı. Bilinen bir immün yetmezliği olmayan ancak emosyonel stres altındaki hastalarda gelişen zona enfeksiyonlarında SSS manifestasyonlarının gelişebileceği akılda tutulmalıdır

Zona zoster associated with aseptic menengitis and hepatitis in two immunocompetent cases after emotional stress

Abstract

Zona zoster is a mostly benign disease in immunocompetent individuals. On the other hand among immunocompomised persons, there is a risk of dissemination. Zona Zoster causes some neurological complications such as myelitis, aseptic meningitis, encephalitis and ventriculitis in both immunocompetent and immunocompromised individuals. Two Twenty and twenty two-year old immunocompetent patients were admitted to emergency service at different time with the similar complaints with fever, severe headache, vomiting and somnolans. In addition one of them had a walking and balance problem. Neck stiffness and meningeal irritation signs were found in both of them. Then we performed a lumbar puncture. Pleocytosis and protein elevation were detected in cerebrospinal fluid CSF samples, respectively. After hospitalization both of their histories were the same, having intense emotional stress before the symptoms had occured. We diagnosed zona zoster with complications from aseptic meningitis according to the clinical findings and positive of serum VZV IgM by ELISA. They cured without sequele after the administration of acyclovir. zoster tanısı konuldu.
