HbA2 ölçümü için BioRad D-10TM ve Tosoh HLC 723 G8 HPLC sistemlerinin karşılaştırılması

Abstract

Amaç: Özellikle Akdeniz bölgesinde önemli bir halk sağlığı sorunu olan hemoglobin bozuklukları taramasında hemoglobin sub tiplerini ayırt etmek amacıyla en yaygın kullanılan yöntem HPLC High Performance Liquid Chromatography ’dir. Pek çok marka ve model HPLC sistemleri ile hemoglobinopati taraması yapılmakta ancak sistemlerin birbirleri ile uyumu bilinmemektedir. Bu çalışmanın amacı; D-10TM Hemoglobin testingsystem BioRad,Hercules,USA marka HPLC cihazı ile Tosoh HLC 723 G8 TosohBioscience, Japan HPLC cihazını hemoglobinopati tanı ve taraması açısından önemli olan HbA2 parametresi ölçümü bakımından karşılaştırılmasıdır. Yöntemler: Çalışmaya Antalya Eğitim ve Araştırma Hastanesi Merkez Laboratuvarı Biyokimya Bölümüne 1-31 Ocak 2016 tarihleri arasında talasemi taraması için başvuran 12’si erkek, 18’i kadın toplam 30 kişi alındı. Her iki cihazda tek seferde çalışılan hemoglobin A2 HbA2 değerlerinin farkları için bağımlı iki grup ve iki ölçüm fark testleri, istatistiksel uyum ve ilişki analizi için iki-yönlü rastgele sınıf içi korelasyon katsayısı SKK , Cohen’s kappa katsayısı, korelasyon katsayısı r ve Bland Altman Analizi yapıldı. Sonuçlar uygun grafiklerle sunuldu. Bulgular: Analizler sonucunda Tosoh HLC 723 G8 cihazında örneklerin 14’ü %46.66 ve BioRad D-10TM cihazında örneklerin dokuz’u %30 β -talasemi taşıyıcısı olarak saptandı. Comparison of the BioRad D-10TM and Tosoh HLC 723 G8 HPLC instruments for the detection of HbA2

Abstract

Objective: HPLC High Performance Liquid Chromatography is the most commonly used method to differentiate the subtypes of hemoglobin in screening for hemoglobinopathy which is a major public health care problem especially in Mediterranean region. Today, different models and trademarks of HPLC equipment have been used in screening for hemoglobinopathy. However, the agreement of the results of the analyses performed by these types of equipment are unknown. In this study, the aim was to investigate the agreement between the results of the analyses of HbA2 values with D-10TM Hemoglobin testing system BioRad,Hercules,USA and Tosoh HLC 723 G8 TosohBioscience, Japan trademark. Methods: A total of 30 individuals, 12 males and 18 females, applied to the Antalya Training and Research Hospital laboratory between January 1 and January 31, 2016 for thalassemia screening were included to this study. Statistical correlations of hemoglobin A2 HbA2 values, analyzed in each analyzer as a single analysis from each sample, were evaluated using two-way random intraclass correlation coefficient ICC , Cohen’s kappa coefficient, correlation coefficient r and Bland Altman analysis. Results are presented with graphics. Results: As a result of the analysis, 14 of the samples 46.66% were detected as – thalassemia carrier with Tosoh HLC 723 G8 system and nine of the samples 30% were detected as – thalassemia carrier with BioRad D-10TM system.
