Antinükleer Antikor-HEP-2 ANA testinin tarama titresi için pozitiflik değerinin belirlenmesi

Abstract

Amaç: ANA sağlıklı kişilerde pozitif gözlenebilmekte ve dilüsyon titresi arttıkça sağlıklı kişilerde görülme sıklığı azalmaktadır. Son yıllarda, dünyada 1:160 tarama titresinin kullanımı yaygınlaşmaktadır. Özellikle CDC ABD Hastalık Kontrol ve Önleme Merkezi ve WHO Dünya Sağlık Örgütü ’nun 1:160 titresini önermesinden sonra kullanımı artmıştır. Ülkemizde ise 1:100 tarama titresi kullanılmaktadır. Bu çalışmada amacımız; ülkemizde 1:160 tarama titresinin kullanımının uygunluğunu araştırmaktır. Yöntem: Çalışmamıza; Acıbadem hastanelerine kan bağışı yapmak amacıyla Ocak - Şubat 2015 tarihleri arasında başvuran ve hazırlanan “Onam Formunu” onaylayan 200 sağlıklı kan dönörü ile birlikte aynı tarihlerde Acıbadem hastanelerinde oto immün hastalık tanısı yeni almış veya takipte olan ve söz konusu Formu onaylayan 200 hasta olmak üzere toplam 400 hasta katılmıştır. Donör ve hastalardan alınan serum numunelerinden ANA testi 1:100 ve 1:160 dilüsyonlarında çalışılmıştır. Donör ve hastalardan yapılan ANA Hep-2 testi sonucunda testin duyarlılık ve özgünlüğü hesaplanmıştır. Testin optimum cut-off değeri, ROC curve analizi ile belirlenmiştir

Determination of cut off level for screening titer of antinuclear antibody-hep-2 test ANA

Abstract

Objective: It is important to choose the correct screening titer in immunoflourescent assay for ANA. Recently, the use of 1:160 titer became widespread in the world, particularly after the suggestion of CDC Centers for Disease Control and Prevention and WHO World Health Organisation . In our country, 1:100 is the mostly used titer for screening as a cut off point. In this study, our aim was to investigate the convenience of suggested 1:160 screening titer for our country.Methods: In order to donate blood 200 out of 400 patients who are healthy blood donors confirmed consent form and applied to between January and February 2015. The rest of the patients has been diagnosed or followed autoimmune rheumatic diseases for the first time and approvaled the form. ANA test which is taken from the serum samples of the donors and patients worked by 1:100 and 1:160. It was figured out the sensitivity and specificity of the ANA as as result of ANA Hep-2 test taken from by donors and patients.
