Bayburt il merkezinde Shigella sonnei gastroenterit salgını, Ekim 2014

Öz

Amaç: Bayburt Halk Sağlık Müdürlüğü 03-11 Ekim 2014 tarihleri arasında 23 kişinin hastaneye yatırılındığını, 794 kişinin etkilendiği gastroenterit vakalarında hızlı bir artış olduğunu bildirmiştir. Bu inceleme, hastalığın kaynağının ve bulaş yolunun tespit edilmesi ve kontrol önlemlerinin alınması amacıyla yapılmıştır. Yöntem: Bu vaka-kontrol çalışmasında hastane kayıtları incelenmiştir. Şüpheli vaka 3-11 Ekim tarihleri arasında gastroenterit ilişkili ICD-10 kodu ile K-52 tanı almış Bayburt ilinde yaşayan kişilerdir. Olası vaka günde üçten fazla ishali olan şüpheli vakalardır. Rastgele seçilmiş olası vakalar ve bunların komşuları olan kontrol grubu karşılaştırılmıştır n=247/247 . Düzenlenmiş tahmini rölatif risk TRRadj su tüketimi ve hijyen alışkanlıklarını içeren lojistik regresyon modeli kullanılarak elde edilmiştir. Etkeni tespit etmek için 32 su numunesi ve 10 gaita numunesinden kültür yapılmıştır. Su depoları ve mahalle çeşmeleri incelenmiştir. Bulgular: Atak hızı ‰13,0’tür 794/60980 . En sık görülen semptomlar ishal %100,0 , karın ağrısı %93,9 , ateş %81,0 ve bulantı’dır %74,5 . Sadece mahalledeki çeşme suyunun içme referans alındığında ve hijyen alışkanlıkları kontrol edildiğinde sadece musluk suyu içme 73/247 , vakalarda kontrollere göre 2,3 kat fazladır TRRadj 2,3,%95 GA 1,4-3,6 . Üç gaita örneğinde S. sonnei tespit edilmiştir. Su deposundan alınan 6 su örneği Coliform 4-13 CFU/100ml , Escherichia coli E. coli 1-2 CFU/100ml , Enterococcus 2-5 CFU/100 ml açısından pozitif bulunmuştur. Tüm örneklerde serbest klor seviyesi 0 sıfır ppm tespit edilmiştir. Çevresel incelemeler su depolarının fiziksel şartlarının klorlama cihazı bulunmaması, paslanmış boru mevcudiyeti gibi uygunsuz olduğunu göstermiştir. Sonuç: Bu S. sonnei salgınının kontamine şebeke suyu kaynaklı olduğu düşünülmektedir. Su depolarının onarımı ve tüm depolarda klorlama cihazı bulundurulması önerilmiştir. Halk Sağlığı eylemi olarak hijyen uygulamaları ve kaynatılmış su tüketimi konusunda halk bilgilendirmiştir.

A Shigella sonnei gastroenteritis outbreak in Bayburt Province, Turkey, October 2014

Abstract

Objective: During 03-11 October 2014, Bayburt Provincial Health Directorate reported a sharp increase in gastroenteritis cases affecting 794 people, 23 hospitalized. This investigation was conducted to identify cause and mode of transmission and to implement control measures. Methods: In this case-control investigation; ospital records were reviewed. A suspected case was a Bayburt resident diagnosed with a gastroenteritis-related ICD-10 code K52 during 3-11 October. A probable case was a suspected case with diarrhea ≥3/day . Exposures of randomly selected probable case-patients with neighborhood control-persons were compared 247: 247 . Adjusted odd ratios ORadj were obtained through logistic regression model including hygiene habits and water consumption. Culture methods to identify pathogens in ten clinical specimens and 32 water samples were used. Water storage tanks and neighborhood fountains were examined. Results: Attack rate was ‰13 794/60980 . Main symptoms were diarrhea %100 , abdominal pain %93,9 , fever %81,0 and nausea %74,5 .When drinking only fountain-water was taken as reference, and after controlling for hygiene habits, drinking only tap water 73/247 was 2,3 times higher in case-patients ORadj=2,3, 95%CI: 1,4-3,6 compared with the controlpersons 55/247 . S. sonnei was detected in three stool specimens. Six water samples taken from water storage tanks were tested positive for total Coliform 4-13 CFU/100ml , E. coli 1-2 CFU/100ml , Enterococcus 2-5 CFU/100 ml . Free chlorine levels were 0 zero ppm in all samples. Environmental investigation showed that water storage tanks were in a bad condition none water chlorination device, rusted infrastructure . S. sonnei was identified in three of ten clinical specimens. Conclusion: This S. sonnei outbreak was likely due to drinking contaminated tap water. We recommended rehabilitation of the water storage tanks and providing chlorination devices for all tanks. As public health action, public were educated on hygiene practices and consumption of boiled water.
