HIV enfekte ergen bir hastada sekonder sifiliz

Öz

Giriş: Son yıllarda dünya genelinde ve ülkemizde, adolesan dönemde insan immün yetmezlik virüsü human immunodeficiency virus, HIV enfeksiyonlarında artış gözlenmektedir. Ülkemizde toplam 16.644 HIV enfekte hastanın 517 %3.1 ’si 19 yaşın altındaki çocuklardan oluşmaktadır. Çocuk verileri kendi içinde değerlendirildiğinde çoğunluğu erkek olmak üzere olguların %61.7’sinin 15-19 yaş aralığındaki ergenler olduğu dikkat çekmektedir. Bu hasta grubunda sifiliz başta olmak üzere cinsel yolla bulaşan diğer hastalıklar da saptanabilmektedir. Bu çalışmada HIV-sifiliz koenfeksiyonu olan 16 yaşında erkek hasta sunulmuştur.Olgu: Tekrarlayan oral aft ve halsizlik nedeniyle tetkik edilirken HIV enfeksiyonu tanısı alan, kliniğimizce takip edilen adolesan hasta iki gündür devam eden düşük dereceli ateş ve makülopapüler döküntü ile prezante oldu. Başvurusunda genel durumu iyi, bilinci açık ve koopere olan hastanın boy-kilo persantilleri yaşına uygundu. Ağız içinde aftöz lezyon saptanmadı. En büyüğü 1.5 cm çapında, hareketli, birkaç adet bilateral servikal ve sağ aksillada saptanan 1 cm hareketli lenfadenopati dışında fizik muayenede özellik yoktu. Hepatosplenomegali palpe edilmedi. Ekstremitelerde belirgin olmak üzere tüm vücudunda yoğun makülopapüler döküntü mevcuttu. A HIV infected adolescent with secondary syphilis

Abstract

Objective: Human immune deficiency virus HIV infection incidence among adolescents has been rising, both globally and in our country in recent years. Approximately 3.1% 517 of 16644 patients with HIV infection is under age of 19 in our country. When children data are seperately considered, it is appearent that 61.7% of those children, mainly males, are between the ages of 15-19. Other sexually transmitted diseases like syphilis can also be encountered in these patients. Here in we present a 16 year old male with HIV-syphilis coinfection.Case: On admission; he was conciouss, well in appeareance, with height and weight percentiles appropriate for age. There was no aphtous lesions in his mouth. He had bilateral cervical multiple lymphadenopathies with maximum diameter of 1.5 cm plus right axillary 1 cm mobile lymphadenopathy. No hepatosplenomegaly was palpated. He had widespread maculopapular rash, which was more intense on extremities. Genital examination was compatible with pubertal male development, it revealed neither lesion nor inguinal lymphadenopathy. Laboratory examination revealed white blood cell count as 1800/mm3 absolute neutrophile count: 800/mm3, absolute lymphocyte count: 900/mm3 . Liver transaminases and renal function tests were within normal range. C-reactive protein was Dünya Sağlık Örgütü DSÖ raporuna göre; 2016 yılının sonu itibariyle yaklaşık 36.7 milyon birey, insan immün yetmezlik virüsü human immunodeficiency virus, HIV ile enfektedir. Aynı yıl içinde yaklaşık 1.1 milyon olgu HIV ilişkili komplikasyonlar sebebiyle hayatını kaybetmiştir. Dünya genelindeki HIV enfekte çocuk sayısı yaklaşık 3.3 milyon civarındadır 1 .
