Sert damakta gelişen pleomorfik adenoma: vaka sunumu

Abstract

Pleomorfik adenoma epitelyal ve mezenşimal kaynaklı benign bir tükürük bezi tümörüdür. Sıklıkla parotis ve submandibular tükürük bezlerinde görülür. Daha nadir olarak ise sert damak ve oral mukozada yerleşimli minör tükürük bezlerinde gelişir. Vakaların %25’inin malign dönüşüm gösterdiği rapor edilmiştir. Pleomorfik adenomanın tedavisinde minör tükürük bezinin geniş eksizyonu yapılır. Kemik tutulumu olan vakalarda ise periost ve ilgili kemik doku da rezeke edilmelidir. Tedavide yetersiz rezeksiyon lokal rekürrense yol açar. Yeterli rezeksiyon sağlanamayan vakalarda cerrahi sonrası radyoterapi gerekli olabilir. Bu vaka raporunda, sert damakta minör tükürük bezinden gelişen bir pleomorfik adenoma vakasının klinik tanısı ve cerrahi tedavisinden bahsedilecektir.

Pleomorphic adenoma arising from hard palate: case report

Abstract

Pleomorphic adenoma is a benign tumor of the salivary glands that has elements of both epithelial and mesenchymal tissues. The tumor most commonly arises in the parotid or submandibular glands. Infrequently, it may arise from the minor salivary glands localized in the hard palate and other parts of oral mucosa. Approximately 25% of cases undergo malignant transformation. Treatment of pleomorphic adenoma is surgical resection with a surrounding cuff of normal tissue. The excision should include periosteum or bone if these are included. If complete resection cannot be achieved, adjuvant radiotherapy should be added. In this case report, we will discuss the clinical diagnosis and surgical treatment of a pleomorphic adenoma arising from minor salivary glands localized in the hard palate.
