Palyatif Bakımda Manevi Danışmanlık ve Rehberlik Hizmetlerinin Psiko-Teolojik Temelleri

Abstract

Bu makalenin amacı, palyatif bakım sürecinde sunulan manevi danışmanlık ve rehberlik hizmetlerini, yaşamın son evresinde ortaya çıkan çok boyutlu ihtiyaçlar bağlamında psiko-teolojik bir perspektiften teorik olarak temellendirmektir. Dolayısıyla bu makalede, palyatif bakım hizmetlerinin yalnızca biyolojik ya da klinik değil, aynı zamanda psiko-sosyo-teolojik düzeyde çok disiplinli bir müdahale alanı sunduğu kabulünden hareketle; bireyin ölümle yüzleşme sürecinde karşılaştığı varoluşsal sorgulamalar, manevi kırılmalar ve anlam boşluklarının nasıl yeniden yapılandırılabileceği derinlemesine ele alınmıştır. Makale kapsamında, palyatif bakımın teorik çerçevesine, Türkiye’deki kurumsal palyatif bakım uygulamalarına, ekip temelli manevi danışmanlık ve rehberlik modelinin yapısına ve psiko-teolojik yaklaşımlarla şekillenen spiritüel müdahale alanlarına odaklanılmıştır. Bu çerçevede, sağlık sisteminin kurumsal yapıları içerisindeki çok disiplinli palyatif bakım ekibi yaklaşımında manevi danışmanın rolü, farklı ülkelerdeki uygulamalarla karşılaştırmalı olarak değerlendirilmiş; sahadaki manevi danışmanlık ve rehberlik hizmetleri ile bu hizmetlerin etik ve kültürel boyutları detaylandırılmıştır. Manevi danışmanlık ve rehberliğin palyatif bakım özelinde hastaların inanç sistemine duyarlı, kültürel olarak bütünleşik ve etik ilkelerle uyumlu bir psiko-spiritüel müdahale biçimi olarak yeniden tanımlandığı vurgulanmıştır. Terminal dönem hastalarının manevi dayanıklılık düzeylerinin desteklenmesi, anlam arayışlarının yönlendirilmesi ve metafizik umut yapılarının güçlendirilmesi gerektiği, disiplinlerarası verilerle temellendirilmiştir. Sonuç olarak makalede, palyatif bakımdaki manevi danışmanlık ve rehberlik hizmetlerinin psiko-teolojik temellerinin ‘(i) varoluşsal sorun alanları, (ii) Tanrı veya başkaları tarafından terk edilmişlik duygusu, (iii) Tanrı’ya veya diğerlerine yönelik öfke duygusu, (iv) Tanrı’yla ilişki hakkındaki sorun alanları, (v) çatışan veya sorgulanan inanç sistemleri, (vi) çaresizlik ve umutsuzluk duygusal durumu, (vii) kayıp ve yas, (viii) suçluluk ve utanç duyguları, (ix) uzlaşma duygusu, (x) yalıtılmışlık, (xi) sadece dine özgü nedenler, (xii) dinsel ve manevi mücadele, (xiii) Tanrı tasavvurunun yeniden yapılandırılması, (xiv) günahkârlık duygusu ve tövbe etmek, (xv) affetmek, (xvi) hastalığın teo-terapötik anlamlandırılması, (xvii) ölümün kabulü ve ölüme hazırlık, (xviii) ölüm kaygısıyla başa çıkmak, (xix) ölüm ötesine ilişkin inanç ve tutumların gelişimi, (xx) dinsel ve manevi krizlerin çözülmesi ve inanç ile maneviyatın güçlendirilmesi’ gibi çok boyutlu psiko-teolojik örüntüyle biçimlenen bir zemin üzerine oluştuğu vurgulanmıştır.

Psycho-Theological Foundations of Spiritual Counseling and Care Services within Palliative Care

Abstract

The present article aims to establish a psycho-theological foundation for spiritual counseling and care services offered within palliative care, particularly in response to the multidimensional needs that arise during the terminal phase of life. Building on the premise that palliative care extends beyond biomedical or clinical interventions and constitutes a multidimensional psycho-socio-theological field, the study examines in depth how existential questioning, spiritual ruptu-res, and crises of meaning encountered in the confrontation with mortality may be restructured through spiritually oriented interventions. The article focuses on the theoretical framework of palliative care, institutional practices in Türkiye, the structure of team-based spiritual counseling and care, and the spiritually shaped therapeutic domains informed by psycho-theological approaches. Within this context, the role of the spiritual counselor in multidisciplinary palliative care teams embedded in institutional health systems is evaluated comparatively across different countries, and the ethical and cultural dimensions of field-based spiritual counseling and care practices are elaborated. The article underscores that spiritual counseling and care, particularly in palliative settings, should be redefined as a psycho-spiritual intervention that is sensitive to patients’ belief systems, culturally integrated, and aligned with ethical principles. Supporting the spiritual resilience of terminally ill patients, guiding their search for meaning, and strengthening metaphysical hope structures are presented as essential components substantiated by interdisciplinary evidence. The study concludes that the psycho-theological foundations of spiritual counseling and care in palliative contexts rest upon a multidimensional therapeutic pattern shaped by: (i) existential concerns, (ii) feelings of abandonment by God or others, (iii) anger toward God or others, (iv) concerns regarding one’s relationship with God, (v) conflicting or challenged belief systems, (vi) states of helplessness and hopelessness, (vii) loss and grief, (viii) feelings of guilt and shame, (ix) experiences of reconciliation, (x) isolation, (xi) religion-specific factors, (xii) religious and spiritual struggle, (xiii) reconfiguration of the God image, (xiv) feelings of sinfulness and re-pentance, (xv) forgiveness, (xvi) attribution of a theo-therapeutic meaning to illness, (xvii) acceptance of death and preparation for dying, (xviii) coping with death anxiety, (xix) the development of beliefs and attitudes about the afterlife, and (xx) the resolution of religious and spiritual crises and the strengthening of belief and spirituality.
