The Evaluation of Ten-Year Laboratory Results of Cyst Hydatid Pre-Diagnosed Patient Samples Analyzed in Microbiology Laboratory of ESOGÜ Medical Faculty Hospital by Age, Gender and Relevant Departments

Abstract

Cystic echinococcosis (CE) is zoonotic metacestode infection caused by Echinococcus granulosus whose adults are identified as hydatid worm and effects herbivorous animals and humans. Diagnosis of CE is based on clinical symptoms, imaging techniques and detection of specific antibodies, especially in patient sera. The aim of this study was to evaluate the relationship between ten years serologic laboratory records of patients with pre-diagnosis of CE according to age, sex and referral departments. Total of 892 patients who were sent to Eskisehir Osmangazi University Faculty of Medicine Microbiology Laboratory with the pre-diagnosis of CE from various clinics in the last decade were studied with E. granulosus IgG ELISA test according to the recommendations of the company. In the evaluation of the results, cut-off and above values were accepted as positive and the experiment was repeated for the limit values and suspicious positives. The results were analyzed with pearson chi-square. Of the 892 CE pre-diagnosed patient’s samples, 482 ones have belonged to females (54%) and 410 have belonged to males (46%) Seropositivity has been found in the ratio of 160(33%) of females and 125(30%) of males (p 0,669). The average of age was divided into two groups with cut-off ratio (35,41) and seropositivity was found to be higher under the age of 35 (p <0,000). According to logistic regression analysis, the risk of developing the disease decreases by approximately 0.01 times with the increase by one unit in age. When the units with a percentage less than 0.5 were excluded for the distribution of the results by their clinics, the p value of the dependency test was found to be p <0,000. Seropositivity was mostly defined in the Polyclinic of Radiology with 68.2% and in the Polyclinic of Chest Diseases in accordance with the distribution of organ involvement (p <0,000). All results are indicated in the tables. Although the incidence of CE depends on age and outpatient variables, the risk was found to be higher in younger ages. No gender differences were observed. Although there are many studies on animals on CE in our country, there is little data on prevalence in humans. The disease is an important zoonosis that continues to affect human health in our province and its surroundings as in our country. The retroperspective study presented here is designed to raise public health awareness.

ESOGÜ Tıp Fakültesi Hastanesi Mikrobiyoloji Laboratuvarına Gönderilen Kist Hidatik Ön Tanılı Hasta Örneklerinin; Yaş, Cinsiyet ve Gönderilen Bölümlere Göre On Yıllık Laboratuvar Sonuçlarının Gözden Geçirilmesi

Abstract

Kistik ekinococcosis (KE) erişkinleri köpek tenyası olarak tanımlanan Echinococcus granulosus’un otçul hayvanlar ve insanlarda oluşturduğu zoonotik metasestod enfeksiyonudur. KE tanısı klinik semptomlar, görüntüleme teknikleri ve hasta serumunda spesifik antikorların saptanması yöntemleriyle konulmaktadır. Çalışmamızda çeşitli kliniklerden KE ön tanısı ile laboratuvarımıza gönderilen ve ELISA yöntemleriyle E. granulosus’a karşı IgG antikor varlığı araştırılan hasta sonuçlarının yaş, cinsiyet ve gönderilen bölümlere göre ilişkisi geriye dönük olarak değerlendirilmiştir. Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Hastanesi Mikrobiyoloji Laboratuvarı’na son on yılda(2009-2019) çeşitli kliniklerden KE ön tanısı ile gönderilen 892 hasta serum örneği Echinococcus granulosus IgG ELISA testi ile firma önerileri doğrultusunda çalışılmıştır. Sonuçların değerlendirmesinde cut-off ve üzeri değerler pozitif olarak kabul edilmiş, sınır değerler ve şüpheli pozitifliklerde deney tekrarlanmıştır. Sonuçların analizi pearson kikare testi ile yapılmıştır. KE ön tanılı 892 hastanın 482’si kadın (%54), 410’u erkek (%46) olup, kadınlarda 160(%33), erkeklerde 125(%30) oranında sero pozitiflik saptanmıştır(p 0,669). Yaş ortalaması (35,41) nokta kesimle hastalar iki gruba ayrılmış ve 35 yaş altında seropozitiflik daha yüksek bulunmuştur( p<0,000). KE ve yaş ilişkisi lojistik regresyon analizine göre, yaştaki bir birimlik artışa karşılık hastalığa yakalanma riski yaklaşık 0.01 kat azalmıştır. Polikliniklere göre sonuçların dağılımları için yüzdesi 0,5’den düşük olan birimler çıkarılınca, bağımlılık testi sonucu p <0,000 bulunmuştur. Seropozitiflik en çok %77’lik dilimle Radyoloji Polikliniği ve organ tutulumundaki dağılıma uygun olarak da Göğüs Hastalıkları Polikliniğinde tanımlanmıştır (p<0,000). Tüm sonuçlar tablolarda belirtilmiştir. KE görülme oranı yaş ve poliklinik değişkenlerine bağlı olmakla beraber, genç yaşlarda riskin daha yüksek olduğu saptandı. Cinsiyete bağlı farklılık istatistiksel olarak anlamlı bulunmadı. KE ile ilgili ülkemizde hayvanlar üzerinde pek çok çalışma bulunmakla beraber, insanlardaki prevalansla ilgili yeterli veriler azdır. Hastalık ülkemizde olduğu gibi ilimiz ve çevresinde insan sağlığını etkilemeye devam eden önemli bir zoonozdur. Burada sunulan retrospektif çalışma halk sağlığına yönelik bir farkındalık yaratmak amacıyla düzenlenmiştir. İlimizde ve çevresinde insan ve hayvan sağlığına yönelik önlemlerin artırılması gerektiği düşünülmektedir
