Abstract

Amaç: Erken bafllang›çl› ve ciddi seyirli epilepsilerde otizm riski bilinmektedir. Ancak West sendromu (WS) ve otizm birlikteli¤i konusunun çok az araflt›r›ld›¤› dikkate al›narak, WS’li hastalarda otistik bozukluk taramas› amaçland›. Gereç ve Yöntem: Çal›flmam›zda 267 hastan›n bilgileri geriye dönük olarak incelendi, ulafl›labilen 90 hastaya Otizm Davran›fl Kontrol Listesi (ODKL) uygulanarak yüksek olas›l›kl› otistik hastalar tespit edildi. ‹ncelenen bulgular ile ODKL’de yüksek olas›l›kl› saptanan veriler bilgisayarda SPSS 12.0 for Windows paket program›nda incelendi. De¤iflkenler ortalama ± standart sapma, standart hata ve yüzde olarak ifade edildi. Verilerin karfl›laflt›r›lmas›nda sürekli de¤iflkenler için Student t-testi, belirtilebilen de¤iflkenler için ki-kare testi kullan›ld›. ‹stanbul T›p Fakültesi Dekanl›¤› Yerel Etik Kurul onay› ( proje no: 2007/ 1289) al›nm›flt›r. Bulgular: Toplam 267 olgunun yafl ortalamas› 56,2±38 ay idi. Olgular›n %80’inde nöbetlerin ilk 6 ayda bafllad›¤› ve ortalama 12 ay izlem sonras› %26,5 ‘inde spazmlar›n durdu¤u görüldü. Ölüm oran› %16,1, psikomotor gerilik %86 idi. Otizm davran›fl kontrol listesi uygulanan 90 olgudan 68 puan alan 17 olguda (%18,9) yüksek olas›l›kla otizm düflünüldü. Bu olgulara 54-67 aras› puan alan 14 olgu da eklendi¤inde oran›n %34,5’e yükseldi¤i görüldü. Yüksek olas›l›kl› otistik olgular›n %94,1’i belirti veren grupta yer al›yordu. Ç›kar›mlar: West sendromunda otistik bozukluk göz ard› edilmeyecek kadar yüksek bir orandad›r. Bu nedenle WS’li olgular otistik bozukluk aç›s›ndan izlemleri s›ras›nda taranmal›d›rlar.
