Modern approach to orthopaedic problems in children – Review

Abstract

There have been a variety of modifications in the treatment of childhood orthopaedic problems in recent years The diagnosis of Developmental Dysplasia of the Hip DDH should be completed with in the first three weeks following birth After six weeks the cases are rather accepted as late The use of double diaper is no longer accepted as a treatment method Pes Equino Varus PEV is treated by 95 using Ponseti Casting Method Pes planus cases except the rigid ones are spontaneously corrected in time Children with spina bifida should be followed up by a multidisciplinary team including a pediatrist After the birth extremities should be kept in functional position First of all the hand eye coordination of children with serebral palsy should be improved For lower extremities if needed surgery is advised to be performed after six years of age Turk Arch Ped 2007; 42: 52 6 Key words: Developmental dysplasia of the hip double diaper pediatric orthopaedics pes equino varus cerebral palsy

Öz

Çocukların ortopedik sorunlarının tedavi yaklaşımları son yıllarda oldukça değişiklikler göstermiştir Gelişimsel kalça displazisi GKD tanısı doğum sonrası ilk üç haftada konulmalıdır Altı haftadan sonra olgular geç kalmış olarak kabul edilmektedir Çift arabezinin tedavide yeri yoktur Pes ekino varus PEV ponseti alçılama yöntemi ile 95 oranında düzelebilmektedir “Rijid” pes planuslar dışındaki pes planuslar zamanla önemli ölçüde gelişim gösterir Spina bifidalı çocuklar çocuk doktoru ile beraber çok merkezli izlenmelidir Doğum sonrası ekstremiteler işlevsel pozisyonda korunmalıdır Beyin felçli çocukların öncelikle el göz uyumu geliştirilmeli cerrahi işlemler mümkün olduğu kadar altı yaşından sonraya bırakılmalıdır Türk Ped Arş 2007; 42: 52 6 Anahtar kelimeler: Beyin felci çift ara bezi çocuk ortopedi gelişimsel kalça displazisi pes ekino varus
