Evaluation of factors predicting the severity of necrotizing enterocolitis in very low birth weight infants Original Article

Abstract

Objective: To determine factors predicting the severity of necrotizing enterocolitis NEC in very low birth weight infants Material and Methods: Fourtyone preterm infants followed in our Neonatal Unit with birth weights lt;1500 gr who developed 53 NEC attacks classified as Stage II and Stage III according to the modified Bell score; were evaluated for the last 5 years retrospectively and compared with regard to the complete blood count and blood glucose values at the presentation of each NEC attack and within 2 3 days of the NEC course Results: Thirtyseven NEC Stage II and 16 NEC Stage III infants were determined The number of male infants were significantly more than girls at Stage III Platelet counts at the beginning and during the NEC attack were lower at Stage III The sensitivity specificity negative and positive predictive values of platelet count lt;135X109 L to determine Stage III cases were 78 62 5 56 and 83 respectively Conclusions: In very low birth weight infants with NEC platelet values could be helpful to predic the severity of the disease Key Words: necrotizing enterocolitis very low birth weight infants severity predictive factors

Öz

Amaç: Nekrotizan enterokolit NEK gelişen çok düşük doğum tartılı yenidoğanlarda hastalık şiddetini öngörecek değişkenlerin belirlenmesidir Gereç ve Yöntem: Son 5 yıl içinde yenidoğan ünitemizde izlenen doğum ağırlığı lt;1500 gr olan 41 preterm yenidoğanda gelişen değiştirilmiş Bell sınıflamasına göre Evre II ve Evre III olarak gruplandırılan 53 NEK atağı geriyedönük olarak dosya kayıtlarından değerlendirildi Her atak anındaki ve 2 3 gün sonrasındaki tam kan sayımı ve kan glikoz değerleri karşılaştırıldı Bulgular: Otuzyedi NEK Evre II ve 16 NEK Evre III atağı saptandı Cinsiyet bakımından Evre III’te Evre II’ye göre belirgin oranda daha fazla erkek olgu mevcuttu Evre III’teki olguların NEK günü trombosit sayısı Evre II’deki olgulara göre daha düşüktü ve NEK atağı boyunca daha düşük trombosit sayılarına ulaştıkları belirlendi Trombosit sayısının lt;135 X109 L olmasının Evre III olgularını saptamada özgüllüğü 78 duyarlılığı 62 5 pozitif tanı değeri 56 ve negatif tanı değeri 83’tü Çıkarımlar: Nekrotizan enterokolit gelişen çok düşük doğum tartılı yenidoğanlarda hastalığın şiddetini öngörmede trombosit sayısı yararlı olabilir
