Surfactant replacement therapy in respiratory failure other than respiratory distress syndrome of prematurity: Report of four cases

Abstract

Many conditions other than respiratory distress syndrome of prematurity causes surfactant deficiency and acute respiratory failure Administration of natural surfactant contributes to improvement of ventilation and is a life saving treatment in these conditions We report four newborns with pneumonia septicemia diaphragmatic hernia and congenital unilobar cystic lymphangiectasis who had surfactant replacement therapy and improvement in oxygenation index mean airway pressure and ratio of arterial alveolar oxygen tension ratio These babies had no pulmonary complications in the follow up period of 12 25 months Congenital unilober cystic lymphangiectasis is the 7th case in the literature and the second case with pre operative surfactant administration; the first case with intraoperative surfactant administration to non lymphangiectazic lung We suggest that surfactant replacement therapy is highly effective in these conditions with high mortality Key words: Surfactant replacement therapy neonate respiratory failure other respiratory distress pneumonia congenital unilobar cystic lymphangiectasis septicemia diaphragmatic hernia

Respiratuvar Distres Sendromu dışı nedenlerle solunum sıkıntısı gelişen dört olguda surfaktan tedavisi

Abstract

Pretermlerde respiratuvar distres sendromu dışında bir çok hastalık da surfaktan eksikliğine yol açarak akut solunum yetersizliğine neden olmaktadır Bu durumlarda surfaktan uygulaması ventilasyonun düzeltilmesine büyük ölçüde katkıda bulunmakta ve hayat kurtarıcı olarak gözükmektedir Pnömoni sepsis diafragma hernisi konjenital tek loba lokalize kistik pulmoner lenfanjiektazi tanılan almış dört yenidoğana surfaktan uygulanarak oksijenasyon indeksi ortalama hava yolu basınçları arteriyel alveolar basınç oranlarında belirgin iyileşme sağlanmış ve1 2 25 aylık izlemlerinde pulmoner komplikasyon gelişmemiştir Kistik unilober pulmoner lenfanjiektazi literatürde sunulan yedinci olgu olup lenfanjiektazik olmayan akciğere intraoperatif surfaktan verilen ilk olgu olma özelliği de taşımaktadır Mortalitesi yüksek olan bu hastalıklarda surfaktan uygulamasının oldukça etkin olduğunu düşünmekteyiz Anahtar kelimeler: Surfaktan tedavisi yenidoğan respiratuvar distres sendromu solunum sıkıntısı pnömoni kistik unilober pulmoner lenfanjiektazi sepsis diafragma hernisi
