Trimethoprim Sulfamethoxazole Amoxicillin Clavulanat and Cefaclor Treatments in Childhood Urinary Tract Infections

Abstract

A prospective randomized study comparing trimethoprim sulfamethoxazole amoxicillin clavulanic acid and cefaclor was conducted on 47 children 12 boys and 35 girls in the treatment of acute urinary tract infection The patient s mean age was 4 7±4 0 years They were treated randomly for 10 days and followed up for at least one month Response to therapy and recurrence rates were evaluated There was no significant difference in the cure rates at 48 72 hours after the initiation and termination of the treatment between the groups On the other hand the recurrence rates were respectively 23 5 in the trimethoprim sulfamethoxazole 69 2 in the amoxcillin clavulanic acid and 42 5 in the cefaclor group within 2 weeks after the termination of the treatment and the high recurrence rate of amoxicillin clavulanic acid group was significant when compared to the trimethoprim sulfamethoxazole group During one year of follow up 20 of 31 patients had recurrence 64 5 and 80 of recurrence occurred within two weeks after the termination of treatment Key words: Bacteriuria Urinary Tract Infection Ultrasonography Voiding Cystoureterography

Üriner Sistem Enfeksiyonlu Çocuklarda Trimethoprim Sulfamethoxazole Amoxicillin Clavulanat ve Cefaclor Tedavi Sonuçlarının Karşılaştırılması

Abstract

Çalışma İstanbul Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Polikliniğinde idrar yolu enfeksiyonu tanısı alan yaş ortalaması 4 7±4 0 olan 35 i kız 12 si erkek 47 hasta ile yürütüldü Hastalara rastgele yöntemle 10 gün süreli trimethoprim sulfamethoxazole amoxicillin clavulanat veya cefaclor tedavisinden birisi verilerek en az 1 ay süreli takip yapıldı ve tedavi yanıtları rekürrens oranları belirlendi Tedavi başlangıcından 48 72 saat ve tedavi bitiminden 48 72 saat sonra başarı oranlarında gruplar arasında anlamlı bir fark olmamasına karşın tedavi bitiminden sonraki 2 3 haftalık izlem sonrasında TMP SMX grubunda 23 5 amoxicillin clavulanat grubunda 69 2 cefaclor grubunda 42 5 oranında rekürrens görüldü Amoxicillin clavulanat grubunda rekürrensin yüksekliği co trimaksazol grubuna göre anlamlı bulundu nbsp; Bir yıl süreyle takip edilen 31 olgudan 20 sinde 64 5 rekürrens olduğu ve bunun 80 inin tedavi bitiminden sonraki 2 hafta içerisinde ortaya çıktığı saptandı Anahtar kelimeler: Bakteriüri İdrar Yolu Enfeksiyonu Ultrasonografi İşeme Sistoureterografisi
