Abstract

Aim: The aim of this study was to evaluate the risk factors for term and pretem newborns etiological agents antibiotic susceptibility and rate of hospital acquired bloodstream infections in newborns hospitalized in our Neonatal Intensive Care Unit during 2008

Bir devlet hastanesinde çok düşük doğum ağırlıklı yenidoğanlarda hastalık ve ölüm oranları

Abstract

Amaç: Hastanemiz yenido¤an yo¤un bak›m birimindeki çok düflük do¤um a¤›rl›kl› hastalar›n hastal›k ve ölüm oranlar›n› belirlemek amac›yla hastalar geriye dönük olarak incelendi. Gereç ve

Yöntem: Hastanemiz yenido¤an yo¤un bak›m biriminde iki y›l içerisinde yatarak tedavi alan çok düflük do¤um a¤›rl›kl› erken do¤mufl bebekler aras›nda çal›flma ölçütlerine uyan 94 olgu çal›flmaya al›nd›. Bu olgular›n verileri (gebelik yafl›, do¤um tart›s›, perinatal risk etmenleri, hastanede yat›fl süreleri, sürfaktan tedavisi, mekanik ventilasyon uygulamalar›, s›k›nt›l› solunum sendromu (SSS), ventriküler içi kanama, periventriküler lökomalasi, nekrotizan enterokolit, kronik akci¤er hastal›¤› ve prematüre retinopatisi) kaydedilip istatistiksel incelemeleri yap›ld›.

Bulgular: Çal›flmam›zda çok düflük do¤um a¤›rl›kl› bebeklerde ölüm oran› %25,5 olarak bulundu. En s›k karfl›lafl›lan anneye ait risk etkenleri preeklampsi/eklampsi (%26,3) olup sonuçlar›m›z “NICHD Neonatal Network” grubunun verileri ile uyumluydu. Çal›flmam›zda grup1’de SSS oran› %100, grup 2’de %93,8, grup 3’te %78,3 ve grup 4’te ise %47,6 olup SSS (+) ve SSS (-) gruplar›n›n cinsiyet da¤›l›mlar› aras›nda istatistiksel olarak anlaml› farkl›l›k gözlenmedi (p=0,19). Ancak erkek bebeklerde SSS (+) olas›l›¤› k›z bebeklerden 1,81 (0,74-4,42) kat riskli bulundu.

Çıkarımlar: Birimimizde % 25,5 olarak belirledi¤imiz ölüm oran›n›n do¤um öncesi bak›m flartlar›n›n düzeltilmesi, steroid kullan›m›n›n artmas›, do¤um öncesi bak›m eksikli¤i nedeniyle s›k karfl›laflt›¤›m›z erken yenido¤an enfeksiyonlar›n azalmas› ve nakil koflullar›n›n düzeltilmesi ile daha iyiye gidece¤i kan›s›nday›z.
