Abstract

Amaç: Çalışmamızda Çocuk Yoğun Bakım Birimi’mize yatan hastalar ileriye dönük olarak izlenerek girişimsel araç ilişkili hastane enfeksiyonu sıklığının saptanması amaçlandı Gereç ve Yöntem: İleriye dönük izlem çalışmasında Bakırköy Dr Sadi Konuk Eğitim ve Araştırma Hastanesi Çocuk Yoğun Bakım Birim’inde Ocak 2008 Temmuz 2009 tarihleri arasında yatan 240 hasta izlendi Hastalıkları Kontrol ve Önleme Merkezi CDC ölçütlerine göre hesaplamalar yapıldı Bulgular: Toplam 240 hastada 2909 hasta gününde toplam 11 girişimsel araç ilişkili hastane enfeksiyonu GAİHE saptandı Genel olarak GAİHE hızı 4 58 GAİHE sıklık yoğunluğu 3 78 1000 hasta günü olarak bulundu Ventilatör kullanım oranı 53 ventilatör ilişkili pnömoni VİP hızı 4 53 1000 ventilatör günü olarak hesaplandı Üriner kateter kullanım oranı 14 üriner kateter ilişkili üriner sistem enfeksiyonu ÜK ÜSE hızı 4 75 1000 üriner kateter günü olarak bulundu Santral venöz kateter kullanım oranı 22 santral kateter ilişkili kan dolaşım enfeksiyonu SK KDE hızı 3 16 1000 santral kateter günü olarak bulundu Enfeksiyon etkenlerine bakıldığında görülme sıklığına göre Coagulase negative stafilococcus 26 09 Pseudomonas aeruginosa 13 04 Escherichia coli 13 04 Staphylococcus aureus 8 70 Candida spp 8 70 Acinetobacter baumannii 4 35 Enterobacter aerogenes 4 35 enterococcus faecium 4 35 Proteus mirabilis 4 35 etken olarak saptandı Seftriyakson sefaperazon ve piperasilin tazobaktam gram negatif bakterilere karşı en etkili ilaçlar olarak bulundu Gram pozitif bakterilerde metisilin direncinin 100 olduğu ve tamamının vankomisine duyarlı olduğu belirlendi Çıkarımlar: Bugünkü veriler ışığında bu çalışma Türkiye’de genel çocuk yoğun bakım birimlerinde yapılan ilk GAİHE etkin izlem çalışmasıdır Birimimizde tespit edilen GAİHE oranları literatür ile uyumlu bulunmakla birlikte daha da düşürmek için çalışmalarımız devam edecektir Türk Ped Arş 2010; 45: 13 7 Anahtar kelimeler: Girişimsel araç santral kateter enfeksiyonu ventilatör ilişkili pnömoni
