Abstract

Congenital dermal sinus is an epithelized tract which may occur anywhere along the spinal axis It may cause meningitis due to the connection between the skin and the medullary canal A case of Escherichia coli meningitis with sacral dermal sinus is presented in this paper Magnetic resonance and ultrasonographic imaging of the lesion showed that the sinus tract had extended into the medullary canal There was also an abscess noted along the sinus tract which had drained into the medullary canal Following the medical treatment with antibiotics the sinus tract was surgically excised Dermal sinus should be carefully searched in cases with meningitis and if present the sinus should be investigated radiologically for possible connection with the medullary canal Key words: Congenital dermal sinus meningitis Escherichia coli

Abstract

Konjenital dermal sinüs vertebral orta hatta spinal aksın herhangi bir yerine yerleşebilen yüzeyi epitelle döşeli bir yoldur Deri ile medüller kanal arasında bağlantı oluşturup sıklıkla görülmeyen mikroorganizmalar ile menenjit ve tekrarlayan menenjit gelişimine neden olabilir Bu yazıda sakral bölgede sinüs ağzı bulunan Escherichia coli menenjiti gelişmiş bir olgu sunulmuştur Dermal sinüs yolunun magnetik rezonans ve ultrasonografik incelemesinde sinüs yolunun medüller kanala uzandığı ve sinüs yolu üzerinde gelişen bir apsenin sinüs yolu aracılığı ile medüller kanala drene olduğu görülmüştür Antibiyotik tedavisi ile menenjit kontrol altına alındıktan sonra operasyonla sinüs yolu çıkartılmıştır Menenjit gelişen her olguda sinüs ağzı dikkatle aranmalıdır Sinüs ağzı saptanırsa sinüsün medüller kanalla ilişkisi radyolojik olarak değerlendirilmelidir Anahtar kelimeler: Konjenital dermal sinüs menenjit Escherichia coli
