Abstract

A 13 years old boy was admitted with hematuria and palpebral oedema On physical examination he was hypertensive and had hepatosplenomegaly Urinalyses revealed proteinuria Serum creatinine C3 and transaminase levels were elevated HB surface antigen antibodies to HB e antigen and Delta Virus were positive Liver biopsy showed chronic active hepatitis and renal biopsy demostrated membranoproliferative glomerulonephritis He was treated with prednisolone for one month Serum creatinine level became normal and proteinuria disappeared A treatment of interferon alfa was planned for 6 months nbsp; In literature no relationship of glomerulonephritis to preceding hepatitis D has been recognized To evaluate this relationship we purpose to determine antibody to D hepatitis in hepatitis B associated glomerulonephropathy Key words: Hepatitis B Virus Hepatitis D Virus Membranoproliferative Glomerulonephritis Chronic Active Hepatitis

Abstract

Onüç yaşında erkek hasta 6 haftadan beri devam eden koyu kırmızı idrar ve göz kapaklarında şişlik yakınmaları ile başvurdu Ödem hipertansiyon ve hepatosplenomegali mevcuttu Makroskopik hematüri proteinüri 1 g gün kreatinin yüksekliği 1 3 mg dl hipokomplemantemi saptandı Transaminazlar normalin iki katıydı HBsAg anti HBe ve anti delta total pozitifti Karaciğer biopsisinde kronik aktif hepatit böbrek biopsisinde membranoproliferatif glomerulonefrit tanısı kondu Bir aylık prednisolon tedavisinin sonunda kan kreatinini 1 mg dl ye düştü ve proteinüri kayboldu Bunu takiben 6 ay süreli alfa interferon tedavisi planlandı nbsp; Literatürde bugüne kadar Delta enfeksiyonu ile böbrek tutulumu arasında ilişki bildirilmemiştir Bu ilişkinin araştırılması açısından Hepatitis B ve böbrek tutulumu olan hastalarda delta enfeksiyonun araştırılması gerektiği düşünüldü Anahtar kelimeler: B Hepatiti Virusu D Hepatiti Virusu Membranoproliferatif Glomerulonefrit Kronik Aktif Hepatit
