Laser posterior cordectomy in bilateral vocal cord paralysis

Abstract

Objectives: We evaluated the functional results of laser posterior cordectomy with respect to respira- tion, phonation, and deglutition.Patients and Methods: Unilateral laser posterior cordectomy was performed in nine patients 7 women, 2 men; mean age 56 years; range 46 to 61 years with bilateral vocal cord paralysis. Postoperatively, respira- tion, deglutition, and phonation were evaluated with the use of an exercise tolerance scale, the Pearson scale, and wavelet analysis, respectively. The mean follow-up period was 14.2 months.Results: Respiration was satisfactory in eight patients. Of these, four patients had excellent, and four patients had good exercise tolerance. The procedure resulted in success without tracheotomy in four patients. No deglutition or aspiration problems developed. Of eight patients with satisfactory respi­ ration, voice quality remained unchanged in six patients, whereas an improved voice quality was noted in two patients who received postoperative phoniatric rehabilitation.Conclusion: Laser posterior cordectomy for the treatment of bilateral vocal cord paralysis resolves respiration problems, it restores phonation and deg­ lutition functions, as well.

İki taraflı vokal kord paralizisinde lazer posterior kordektomi

Abstract

Am a ç : Lazer posterior kordektomi ile tedavi ettiğimiz iki taraflı vokal kord paralizili olguları solunum, fonas- yon ve yutma fonksiyonları açısından değerlendirdik. Hastalar ve Yöntemler: İki taraflı vokal kord parali- zisi saptanan dokuz hastaya tek taraflı lazer posterior kor- dektomi uygulandı. Ameliyat sonrası dönemde, solu- num egzersiz toleransına göre, yutma Pearson ska- lasına göre, fonasyon wavelet analiz sonuçlarına gö- re değerlendirildi. Olgular ortalama 14.2 ay süreyle izlendi. Bulg u l a r : Dokuz hastanın sekizinde yeterli solunum sağlandı. Bu hastaların dördünde çok iyi, dördünde ise iyi derecede egzersiz toleransı elde edildi. Ye t e r l i pasaj sağlanan olguların dördünde cerrahi trakeoto- misiz olarak gerçeklefltirildi. Hiçbir hastada yutma ve- ya aspirasyon sorunu geliflmedi. Yeterli solunumun sağlandığı sekiz hastanın altısında wavelet analizi so- nucunda ses kalitesinde belirgin değifliklik saptan- mazken, uygulanan foniatri eğitimi sonucu iki hasta- nın ses kalitesinde artıfl görüldü. Sonuç: Lazer posterior kordektomi tekniği iki taraflı vokal kord paralizisinde solunum sorununu çözer- ken, fonasyon ve yutma fonksiyonlarının korunması- nı sağlamaktadır
