A case of legionella pneumonia masked by the course of diabetic ketoacidosis

Abstract

Legionella pneumophila is a pathogen that can cause Legionnaires’ disease. Infection with L. pneumophila presents with a clinical manifestation similar to other community-acquired types of pneumonia, characterised by symptoms such as fever, fatigue, shortness of breath, cough and malaise, as well as widespread infiltrates visible on chest imaging. Risk factors include being over 50 years of age, male gender and chronic illnesses. This disease, which resembles community-acquired pneumonia, can lead to septic shock and organ failure. In this case report, Legionella pneumonia was diagnosed in a 69-year-old male patient who presented with nausea and vomiting but no respiratory symptoms, and who had been diagnosed with diabetic ketoacidosis. This case highlights the importance of considering Legionella pneumonia in the differential diagnosis of patients presenting with an atypical clinical picture.

Diyabetik ketoasidoz seyrinde maskelenen legionella pnömonisi olgusu

Abstract

Legionella pneumophila, Lejyoner hastalığına yol açabilen bir patojendir. L. pneumophila enfeksiyonu, ateş, halsizlik, nefes darlığı, öksürük, güçsüzlük ve akciğer görüntülemesinde yamalı infiltrasyonlardan oluşan belirtilerle, toplum kaynaklı pnömoninin diğer formlarına benzer bir klinik sunuma sahiptir. Risk faktörleri arasında 50 yaş üstü, erkek cinsiyet ve kronik hastalıklar yer alır. Toplum kaynaklı pnö-moniye benzeyen hastalık, septik şok ve organ yetmezliğine neden olabilir. Bu olgu sunumunda; solu-numsal yakınması olmayan, bulantı ve kusmayla başvuran, diyabetik ketoasidoz tanısı alan 69 yaşında erkek hastada Legionella pnömonisi saptanmıştır. Bu olgu Legionella pnömonisinin atipik klinik prezentasyonu olan hastalarda ayırıcı tanıda düşünülmesi gerektiğini vurgulamaktadır.
