Postoperatif Pelvik Lenfosellerin Perkütan Etanol Skleroterapi ve Kateter Drenajı ile Tedavisi

Abstract

Amaç: Çalışmanın amacı pelvik lenfosel gelişen hastalarda perkütan transkateter etanol skleroterapi (PTES) yönteminin etkinli ğini ve uzun dönem takip sonuçlarını araştırmaktır. Materyal ve Metot: Bu çalışmaya 1993-2000 yılları arasında pelvik lenfosel tedavisi nedeniyle refere edilen 33 hasta dahil edildi. 33 hastada bulunan toplam 38 lenfosel skleroterapi kullanılarak veya kullanılmadan perkütan kateter drenaj ile tedavi edildi. Lenfoseller jinekolojik malignansi nedeniyle opere edilen 33 hastada (23 over kanseri, 5 serviks kanseri, 5 uterus kanseri) gelişti. Lenfoseller ultrasonografi ve floroskopi kılavuzluğunda seldinger yöntemi kullanılarak perkütan kateterize edildikten sonra 150 mlden az olan lenfokistler aspire edilip bir gün sonra, diğerleri ise drenaj 10 mlnin altına indikten sonra etanol ile skleroze edildi. 7 lenfoselde perkütan etanol skleroterapi uygulanmadı ve sadece perkütan kateter drenaj uygulandı. Hastalar ultrasonografi ile takip edildi. Bulgular: Ortalama lenfosel hacmi 368 ml (15-2900 ml) idi. Ortalama kateterizasyon süresi 10.8 gündü (1-60 gün). Takip süresi ortalaması 21 aydı (2-64 ay). PTES ile tedavi edilen. 31 lenfoselin 28inde (%90) ve sadece perkütan kateter drenajı ile tedavi edilen 7 lenfoselin 4ünde (%57) ilk tedavi ile başarılı sonuçlar sağlandı. PTES ve sadece perkütan kateter drenaj ile tedavi edilen üçer hastada (toplam 6 hasta) nüks gelişti. Bu hastaların dördünde nüks lenfokistler PTES yöntemiyle tedavi edildi ve daha sonra nüks izlenmedi. Sonuç: Perkütan transkateter etanol skleroterapi postoperatif lenfosel tedavisinde etkin ve güvenilir bir yöntemdir.

Abstract

Objective: The aim of this study is to investigate the efficacy and long-term results of percutaneous transcatheter ethanol sclerotherapy (PTES) for postoperative pelvic lymphocyst treatment. Materials and Methods: Thirty-seven patients were included in this study who were referred for lymphocyst treatment between 1993-2000. Thirty-eight lymphocyst of 33 patients were treated by percutaneous treatment with or without ethanol sclerotherapy. Lymphocysts developed in 33 patients, who underwent gynecologic malignancy operation (23 ovarian cancer, 5 cervix cancer, 5 uterus cancer). Lenfoceles were catheterized by ultrasonography and fluoroscopy guidance using seldinger technique. Lymphocysts smaller than 150mL underwent ethanol sclerotherapy the day after catheter drainage and the others were treated when the catheter output decreased below 10mL. In 7 patients percutaneous ethanol sclerotherapy could not be performed and they were treated only by percutaneous catheter drainage. Patients were followed up by ultrasonography. Results: Mean lymphocele volume was 368 mL (15-2900 mL). Mean catheterization duration was 10.8 days (1-60 days). Mean follow up time was 21 months (2-64 months). Initial treatment was successful in 28 out of 31 (90%) lymphocyts treated with PTES and 4 out of 7 (57%) lympocysts treatd with percutaneous catheter drainage. Minor complications (secondary infection and catheter dislodgement) were noted in 6 patients. Recurrence developed in three patients in each group (6 patients) treated by either PTES or percutaneous catheter drainage. Four of these patients were treated with PTES without further recurrence. Conclusion: Percutaneous transcatheter ethanol sclerotherapy is an effective and reliable method for treament of postoperative lymphocyst.
