SEKS KORD STROMAL TÜMÖRÜ OLAN BİR HASTADA FERTİLİTE KORUYUCU CERRAHİ VE KEMOTERAPİ SONRASI GEBELİK

Abstract

Giriş: Over kanserleri üreme çağındaki kadınlarda nadir görülür. Ancak; fertilitesini korumak isteyen bir hasta tanı aldığında hastalığın tedavisi; kanserin evresi; derecesi; yüksek mitotik indeksi; tümör büyüklüğü ve tümörün rüptüre olup olmamasına göre değişiklik gösterir. İyi seçilmiş olgularda yakın sonografik ve hormonal izlem ile adjuvan kemoterapi ile birlikte veya adjuvan kemoterapi olmadan fertilite koruyucu cerrahi planlanabilir. Olgu Sunumu: Seks kord stromal tümörü olan ve ikinci gebeliğini isteyen bir hastayı olgu olarak inceliyoruz. Operasyon öncesi hasta değerlendirildikten sonra; cerrahi evreleme ve uygun cerrahi prosedürün belirlenmesi için laporotomiye alındı. Operasyon esnasında over ca evre 1 olarak rapor edildi ve unilateral salpingo ooferektomi yapıldı. 4 kür adjuvan kemoterapi sonrası hasta gebe kaldı. Sezeryan/ seksiyo ile bir adet sağlıklı erkek bebek dünyaya geldi. Tartışma: Erken evre seks kord stromal tümörü olan ve operasyon esnasında fertilitesini korumak isteyen hastalarda fertilite koruyucu cerrahi sonrası adjuvan kemoterapi uygulanabilir.

Abstract

Objective: Overian cancer occur rarely in reproductive ages but if diagnosed in a patient, want to preserve fertility, management of cancer changes according to cancer stage and other prognostic factors like grade, high mitotic index, tumor size and rupture of tumor. In wellselected patients, fertility sparing surgery may consider with or without adjuvant chemotherapy under close sonographic and hormon monitoring. Case: We report a patient with seks cord-stromal tumor with desire of second baby. After evaluation of patient preoperatively, she underwent laparatomy for surgical staging and obtain unilateral laparotomy for surgical staging and obtain appropriate surgical procedure. Intraoperatively, overian cancer defined as stage I and only unilateral salpingo-oophorectomy was performed. Following adjuvant chemotherapy for four cycles, she became pregnant and healty, male baby was delivered by cesarean section. Intraoperative period, biopsy was taken from contralateral over and pathology was seen normal. Discussion: Fertility sparing surgery, followed by adjuvant chemotherapy can consider for any women who wishes to preserve her fertility durng operation for seks-cord stromal tumors at early stages.
