PELVİK MALİGNANSİYİ TAKLİT EDEN MESANE İNVAZYONU YAPAN KLEBSİELLA PNEUMONİA TUBA OVARYAN ABSE OLGUSU
Abstract
Giriş: Tuboovarian apse genellikle pelvik inflamatuar hastalığın bir sonucudur. Bu yazıda pelvik maliniteyi taklit eden bir tuboovarian abse olgusu sunuyoruz. Olgu Sunumu: 44 yaşındaki kadın pelvik ağrı ve kitle şikayeti ile hastanemize başvurdu. Hastada ateş veya lökositoz yoktu. Yapılan USG'de 150x100 mm. boyutunda karın duvarı ve idrar kesesini infiltre etmiş bir kitle saptandı. MRG'de karın ön duvarı ve uterin fundusu invaze eden düzensiz sınırlı benzer bir kitle tespit edildi. Pelvik malignite şüphesi ile yapılan laparotomi sonucunda tuboovarian apsesinin kesin tanısı konuldu Tartışma: Tuba-overian apse adneksiyal kitlelerin ayırıcı tanısında klinik belirtiler gizlenmiş olan bile dikkate alınması gereken koşullardan biridir.

Abstract
Objective: Tuboovarian abscess is usually a consequence of pelvic inflammatory disease. Hereby we present a case of tuboovarian abscess mimicking malignancy. Case Repeort: A 44 year old woman referred to our hospital for pelvic pain and mass without fever and leukocytosis. USG showed a mass with 150x100 in size that infiltrated abdominal wall and urinary bladder .MRI detected a similar mass with irregular borders that invaded the anterior abdominal wall and uterine fundus. Pelvic malignancy was suspected and laparotomy was performed with a definitive diagnosis of tuboovarian abscess. Conclusion: Tuba-ovarian abscess is one of the conditions that have to be considered in the differential diagnosis of adnexial masses even if clinical symptoms are disguised.
