NADİR GÖRÜLEN BİR GESTASYONEL TROFOBLASTİK NEOPLAZİ FORMU; PLASENTAL SİTE TROFOBLASTİK TÜMÖR VE LİTERATÜRÜN GÖZDEN GEÇİRİLMESİ

Abstract

Plasental site trofoblastik tümör (PSTT) intermediate gestasyonel trofoblastlardan kaynaklanan Gestasyonel trofoblastik neoplazilerden (GTN) bir tanesidir. GTN'lerin nadir görülen bir tipidir ve şimdiye kadar literatürde 300 kadar olgu bildirilmiştir (1). Trofoblastik tümörlerin %1-2‘sini oluşturur (2). Düşük insidansda olması etyoloji ve klinikopatolojisinin anlaşılmasını sınırlandırır. Tipik olarak reproduktif yaştaki kadınlarda görülür ve term gebelikten aylar ya da yıllar sonra ortaya çıkabilir. Fakat herhangi bir gebelikten sonra da ortaya çıkabilir. PSTT‘ye erken evrede tanı koymak kolay değil çünkü hastalar sıklıkla düzensiz vaginal kanama ve amenore ile başvururlar. β-HCG seviyeleri genellikle orta derecede yüksek ve koryokarsinomlu hastalardan daha düşüktür (3). Tanı için intermediate trofoblastlarda proliferatif büyüme myometrial kaslara infiltrasyon gereklidir. Tanı için intrauterin aspirasyon yetersiz, uterin küretaj gereklidir. GTN'ler kemoterapiye duyarlı iken, PSTT'lar zayıf biyolojik davranışından dolayı göreceli olarak kemorezistanstırlar (3). PSTT yavaş büyür, uterus içine yayılır ve lenf nodlarına geç metastaz yapar. Non metastatik hastalığın en etkin tedavisi histerektomidir (3). Yüksek mitotik indeksli büyük tümör volumlü ve metastatik olgularda adjuvan kemoterapi önerilir. Biz bu olgu sunumunda 32 yaşında yüksek B-HCG seviyesi ve düzensiz kanamalarla kliniğimize başvuran bir hastada yapılan tetkiklerde PSTT tanısı alan bir hastayı sunmayı amaçladık.

A RARE GESTATIONAL TROP HOBLASTIC NEOPLASIA FORM; PLACENTAL SITE TROP HOBLASTIC TUMOR AND LITERATURE REVIEW

Abstract

Placental site trophoblastic tumor (PSTT) is one of the gestational trophoblastic neoplasias (GTN) arising from intermediate gestational trophoblasts. It is a rarely seen type of GTN's, and 300 cases have been reported in the literature so far (1). Is also covers 1-2%of trophoblastic tumors (2). Having a low incidence limits the understanding of its etiology and clinical pathology. Typically seen in women of reproductive age and term pregnancy may occur months or years later. It may occur after any pregnancy as well. PTTY is not easy to diagnose in the early stages because patients often present with irregular vaginal bleeding or amenorrhea. β-HCG levels are generally lower than those patients moderately high and choriocarcinoma (3). Infiltration of the proliferative growth myometrial muscle in the intermediate trophoblast is required for diagnosis. İntrauterine aspiration is inadequate and uterine curettage is required for diagnosis. While GTNs are responsive to chemotherapy, PSTTs are relatively chemoresistance due to the weak biological behavior (3). PSTT grows slowly, spreads into the uterus, and makes metastatis passing to lymph nodes. Hysterectomy is the most effective treatment to the non-metastatic disease (3). For metastatic patients and with high mitotic indexed large tumor adjuvant chemotherapy is recommended. We aim to provide a PSTT which is a rare form of GTNs'case accompanying the literature in the case report.
