GEBELİKTE BORDERLİNE EPİTELYAL OVER TÜMÖRÜ: OLGU SUNUMU

Abstract

Bu makalede 18 yaşında bir hastada gebeliğin sekizinci haftasında rutin ultrasonda rastlanan ve yükselmiş Ca-125 düzeyleri ve cerrahi eksplorasyonla tanı konan borderline over tümörü olgusu sunulmuştur. Hastaya gebeliğin onuncu haftasında sol salpingo-ooferektomi ve evreleme cerrahisi uygulanmıştır. 38. haftada hasta sezeryan ile doğum yapmıştır. Hastanın izlemi üçüncü yılında sorunsuz bir şekilde devam etmektedir. Bu tür hastalarda jinekolog onkolog, perinatolog, neonatolog ve psikologdan oluşan multidisipliner yaklaşım önem kazanmaktadır. Reprodüktif dönemde borderline over tümörü olgularında fertilite koruyucu cerrahinin önemi de tartışılmıştır.

Abstract

Ovarian tumors during pregnancy are a rare event. In most cases the tumors are detected accidentally during routine examination, ultrasound or a cesarean section at term. Histologic subtypes and prognosis do not differ from tumors not associated with pregnancy; it seems however, that there are more lesions of borderline malignancy and of low grade. We present the case of an 18 year-old pregnant woman with an ovarian tumor of low malignant potential. A successful delivery with cesarean section occurred when the patient was in 38 week of pregnancy. Her follow-up is still going on with watchful waiting. For the treatment of patients with pelvic masses suspicious of malignant appearance operative therapy should be performed. Fertility preserving surgery is adequate therapy for women of reproductive age with ovarian tumors of borderline malignancy. To determine a better approach for such patients, multidisciplinary staff meetings, including gynecological oncologists, obstetricians, neonatologists, psychologists, and the patient are important.
